EHR and practice managementYour EHR Shows What Was Billed. Does It Show Why Claims Fail?
It documents care, runs the schedule and produces the charge. It was never designed to tell you why one payer denies one service line, or whether that started last quarter. You may not need another EHR. You may need better visibility into your revenue.

What is the difference between EHR reporting and revenue intelligence?
EHR reporting describes what is in the EHR: charges entered, payments posted, balances outstanding. Revenue intelligence analyzes revenue-cycle and remittance data across payers and over time to find patterns the ledger does not express. The first tells you the balance. The second tells you why the balance looks like that.
Your EHR is accurate about the ledger, and it is organized around the encounter. The patterns that matter to revenue are the ones that only show up across many encounters: that one payer has been adjusting one service line differently since a policy change, that a provider's claims fail more often than the group's, that a category of denial keeps coming back after being reworked.
Your EHR Is Doing Its Job. This Is a Different Job.
Practices are often told their software is the problem. Usually it is not. The gap is between systems, in the part of the process that nothing owns.
| The question | Where the answer lives |
|---|---|
| What did we bill this month? | Your EHR or practice management system. |
| What has been posted against it? | Your EHR or practice management system. |
| Was this claim transmitted, and was it accepted? | Your clearinghouse. |
| What did the payer say about it? | The remittance advice that came back. |
| Why did this particular claim fail? | A claim-level investigation. VixisTrace™. |
| Is the same thing failing repeatedly, and where? | Analysis across claims and payers. VixisIQ™. |
| Who is going to do something about it, and when? | Revenue management. That is a team, not a report. |
This is why CareVixis does not open by asking a practice to replace software it is happy with. Changing the EHR does not answer any of the last three questions. Something has to be responsible for them.
Where the Data Comes From
An example of the path revenue-cycle data takes from the EHR to analysis. This is a conceptual illustration of the general flow. It is not a description of any specific practice's configuration, and the exact arrangement is confirmed per practice.
- Your EHR or practice management systemThe encounter is documented and the charge is created.
- Claims and revenue-cycle workflowCoding, claim preparation and submission.
- Clearinghouse and payerThe claim is transmitted and translated where needed, then adjudicated.
- Remittance information returnsPayment, adjustment, denial and patient responsibility detail, with the payer's reason codes.
- VixisIQ™Where appropriate data can be obtained and normalized, the remittance and revenue-cycle information is analyzed for patterns.
- VixisTrace™, when a claim needs a root causeIndividual problem claims are investigated rather than only reworked.
- Actionable revenue intelligenceFindings, what they appear to mean, the action taken and who owns the next step.
Find Your Platform
Grouped by the kind of practice each platform serves, because the revenue-cycle question differs far more by practice type than by software brand. Platform names are listed to help you find your situation.
Behavioral health and human services
Recurring, time-based services, authorization limits on ongoing therapy, telehealth place of service, and credentialing across many payers for many clinicians. Denials cluster around authorization units, eligibility on long-running cases and provider enrollment dates.
Platforms practices tell us they run: TherapyNotes, SimplePractice, Valant, ICANotes, TheraNest, Osmind, BestNotes, Kipu, Netsmart, Qualifacts.
Primary care and ambulatory
High claim volume, wide payer mix, preventive and chronic care coding, and annual wellness and screening rules that change. Small adjustment patterns matter here because they repeat across thousands of encounters.
Platforms practices tell us they run: athenahealth, eClinicalWorks, NextGen Healthcare, Tebra, Practice Fusion, AdvancedMD, Greenway Health. Tebra was formed in 2021 by the merger of Kareo and PatientPop, so a practice may still call it Kareo.[2]
Specialty practices
Procedure-heavy billing, modifier accuracy, global periods, implants and supplies, and prior authorization on imaging and procedures. The same denial reason can mean something completely different from one specialty to the next.
Platforms practices tell us they run: ModMed, Nextech, Compulink, Eyefinity, RevolutionEHR, PatientNow, Raintree, Experity, PCC.
Therapy and rehabilitation
Visit limits, plan of care and recertification requirements, therapy modifiers and units billed in timed increments. Documentation requirements drive a large share of the problems.
Platforms practices tell us they run: WebPT, Prompt, Raintree.
Enterprise and health system
Practices inside or affiliated with a health system, where the EHR is set by the organization and the practice still carries the revenue consequences.
Platforms practices tell us they run: Epic, Oracle Health. Oracle completed its acquisition of Cerner in June 2022 and the business became Oracle Health, so both names are still in circulation.[3]
Home health, hospice and post-acute
Episodic payment, eligibility and certification periods, and visit verification requirements. A single missed certification can invalidate a series of claims.
Platforms practices tell us they run: WellSky, MatrixCare, Netsmart.
Why there is not yet a page for every platform
A page per vendor is easy to generate and mostly worthless. CareVixis publishes a platform-specific page when it can carry researched, sourced detail about that platform's workflow and the revenue-cycle questions it raises, and not before. Platform pages are being published in stages.
If your platform is not listed, that is not an exclusion. Ask. The question that matters is what revenue-cycle data can be obtained, not which logo is on the login screen.
How to Choose a Medical Billing Company When You Already Have an EHR
Ten questions, written to be asked of any billing or revenue-cycle company, CareVixis included. The point is that a weak answer to any of them is easy to hear.
-
Can they work with your existing EHR, and what exactly do they need from it?
A specific answer names the data. A company that says it works with everything has not checked, and you will find out during implementation.
-
Will they require you to move to their software?
Ask whether keeping your EHR is genuinely supported or tolerated, and whether the price changes if you keep it. With CareVixis, keeping your own EHR carries no monthly software fee.
-
Do they understand your specialty's reimbursement?
Ask about the authorization rules, coding families and documentation requirements that actually apply to you. Generic billing competence is not the same thing.
-
Who works denials, and what happens on the third repeat?
Rework recovers a claim. Root cause analysis is what stops the fourth one. Ask which one you are buying.
-
Do they analyze remittance data, or only post from it?
The remittance advice is a standardized transaction, so it can be analyzed across payers and over time. Posting from it and analyzing it are different activities.
-
How is A/R worked, and who decides to write something off?
Ask at what age a balance gets attention, who is told before a write-off, and who watches filing limits.
-
What will you be able to see without asking?
Ask what visibility you get and in what form. Then ask what would have alerted you to a claim that has not moved in 60 days.
-
Is any part of the work performed offshore?
Ask about subcontractors as well as employees. CareVixis is 100% US based and does not outsource.
-
What extra technology are you expected to license?
If the reporting in the demonstration depends on a product you have to buy separately, that belongs in the price comparison.
-
Who is accountable when something goes wrong?
Ask for the name, and ask how you reach a decision maker. CareVixis offers direct access rather than a queue.
Already Comfortable With Your EHR? Good.
CareVixis does not need to begin a relationship by asking a practice to abandon technology that works for it. Your staff know that software. Your workflows are built around it. Replacing it has a real cost, and it answers none of the questions that brought you here.
Keeping your system is a supported outcome, not a fallback. If a practice later wants to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis, so the decision is about what you want, not about price.

Questions Practices Ask About Their EHR and Billing
Can I keep my EHR and still change billing companies?
Yes. The billing and revenue-cycle work is a separate decision from the software that documents care. Practices change billing companies while keeping the same EHR regularly. CareVixis treats keeping your EHR as a supported outcome, and keeping it carries no monthly software fee from CareVixis.
Does CareVixis have an integration with my EHR?
CareVixis does not claim an API integration, native integration, certification or partnership with any EHR vendor unless that relationship is documented. CareVixis is designed to work alongside existing healthcare technology, and what can be obtained from a given system is confirmed for that practice before any commitment.
Why are my claims being denied if my EHR says they were submitted?
Submission and adjudication are different events. A claim can leave the EHR correctly, be rejected before adjudication for a data problem, or be adjudicated and denied on the payer's policy. The EHR usually shows that it was sent. The reason it failed is in the payer's response, which is what claim root cause analysis examines.
Do I need to buy more software to get revenue reporting?
Not from CareVixis. VixisIQ™ is CareVixis technology and a practice is not asked to license a third-party analytics product to see its own revenue data. Any optional CareVixis service is named and priced before it starts.
My EHR vendor also offers billing. Why would I use anyone else?
Sometimes you should not: if it works and you can see what is happening, keep it. The questions worth asking are the ones in the framework above, particularly who investigates a repeat denial and what you can see without asking. Judge any provider, including CareVixis, on those answers.
What if my practice runs more than one system?
That is common, and it is usually where visibility breaks down, because no single system sees the whole path. It is worth saying up front so the data scope is established properly rather than discovered later.
