What Does It Really Cost to Run Your Practice?
Adjust the numbers below to compare the cost of assembling your own systems, vendors and administrative support with one complete CareVixis practice system.
Do You Need the Whole Jet, or Just the Seat?
Most providers do not need to own an entire practice infrastructure any more than a traveler needs to own a jet. When you fly, you purchase a seat while the airline provides the aircraft, pilots, maintenance, systems and support. CareVixis has already invested in the technology and specialized teams required to operate a modern practice. Because those resources support multiple providers, your practice receives the benefit of the complete system while paying only for the capacity it uses.
If you need to take a trip, you do not purchase an aircraft, hire pilots, employ a maintenance crew, rent a hangar and manage aviation regulations. You purchase the capacity you need while the airline operates the infrastructure.
A modern healthcare practice works the same way. You can separately purchase an EHR, clearinghouse, billing support, credentialing, a website, SEO, communications and administrative labor. You must then make all those pieces work together.
CareVixis has already built and staffed that infrastructure. Because it supports multiple securely separated practices, you pay for the capacity your practice uses instead of financing the entire system yourself.
What You Always Own and Control
- Your practice
- Your brand
- Your patient data
- Your payer contracts
- Your clinical decisions
- Your business direction
You retain ownership of your practice, brand, client relationships and data. CareVixis provides and manages the infrastructure supporting them.
Adjust the Numbers for Your Practice
Defaults represent a typical single-provider practice. Every slider updates all results instantly.
Practice Profile
Basic volume and revenue inputs for your practice.
Billable providers, including physicians, NPs, and PAs.
Total billed amount before contractual adjustments and payer reductions.
Payer allowable as a percentage of gross charges. Most practices range 55-80%.
Percentage of claims denied on first submission. Industry average is 5-15%.
Of claims initially denied, what percentage do you ultimately recover? Without active denial management, most practices recover 40-60%.
Deductibles, copays, and coinsurance as a share of the contracted allowable amount.
Percentage of patient responsibility that goes uncollected. National average is 10-25%.
CareVixis includes credentialing for up to 18 payers. Additional payers are available at a one-time fee.
Cost and Time Inputs
Independent vendor costs and administrative time estimates. Set any item to $0 if your practice does not currently use it.
Typical standalone billing companies charge 4-10% of collected billing. This fee does not include EHR, credentialing, website, or other systems.
One-time cost to credential each provider with each insurance payer. Services typically charge $200-$500 per payer per provider. Total is multiplied by your number of providers and payers.
Ongoing cost to keep credentials current: re-attestation, CAQH updates, license renewals, and responding to payer re-credentialing requests.
Monthly subscription cost per provider for your electronic health record system. Major platforms range from $50 to $500+ per provider.
Business phone lines, VOIP system, or virtual phone service. Does not include personal cell phone usage.
Automated appointment reminders, payment reminders, and patient messaging platforms.
One-time cost to build or redesign your practice website. Set to $0 if you do not currently have a website.
Hosting fees, SSL certificates, security updates, content updates, and general website maintenance.
Ongoing cost of retaining a separate SEO or digital marketing agency. Set to $0 if you are not currently investing in SEO.
Cost of managing social media accounts (Facebook, Instagram, Google Business Profile, etc.) either in-house or through an agency.
Hours spent by providers or senior staff on billing follow-up, vendor coordination, and administrative tasks each month.
What is an hour of your time worth when spent seeing patients or building the practice? Used to calculate the opportunity cost of administrative burden.
If your back-office were fully managed, what portion of current admin hours could realistically shift to patient care or practice growth?

The Hidden Cost Nobody Counts
Every calculator that compares billing fees ignores the most expensive line item in most practices: provider time spent on administration.
When you are coordinating between a billing company, an EHR vendor, a clearinghouse, a credentialing service, a website agency, and a phone system provider, you are the integration layer. You are the one who receives the finger-pointing when something breaks between systems. You are the one who spends 20 minutes on hold to learn that the denial came from data missing in a field that two vendors say is each other's responsibility.
The calculator above uses a conservative estimate. You can adjust both the number of admin hours and your value per productive hour to see the opportunity cost specific to your situation. Recapturing even 25% of that time through a fully managed back-office changes the economics of the comparison significantly.
This is separate from the direct cost savings. It is an additional reason the total value of a unified system is almost always larger than the fee comparison alone suggests.
Buying Billing Software Does Not Work the Denials
One of the most common misconceptions in practice management is that purchasing billing software solves the denial problem. Software submits claims. It does not follow up on them. The denial management problem is a people, process, and persistence problem, not a technology problem.
Understanding what you are actually dealing with matters. Not all payment reductions are the same thing:
Contractual Adjustments
The difference between gross charges and the contracted allowable. This is expected and cannot be appealed. The calculator accounts for this automatically using your allowable percentage.
Rejections
Claims returned before processing, usually for missing data or formatting errors. Rejections are correctable and must be resubmitted promptly to stay within timely filing deadlines.
Denials
Claims processed but not paid, with a reason code. Most denials are appealable. The recovery rate in this calculator reflects how many get worked versus written off.
The denial recovery rate in the calculator is the variable that most directly reflects the quality of your current billing operation. If you are recovering less than 70% of denied revenue, a significant portion of your earned income is being permanently abandoned. Most practices using independent billing companies without active denial management recover 40-60%. CareVixis targets 90%+ with its proprietary denial tracking engine and dedicated follow-up process.

Fixed configuration constants (not adjustable via sliders): clearinghouse $50/month, eligibility and denial support $500/month, systems setup $500/provider. All other independent cost inputs (credentialing, EHR, phone, SMS, website hosting, SEO, social media) are adjustable above. Set any to $0 if your practice does not currently use that service.
- Software prices used are illustrative planning assumptions based on publicly available rate ranges. Actual vendor pricing may differ.
- Billing companies commonly use percentage-of-collections pricing. The default independent rate of 6% represents a mid-range industry estimate.
- Credentialing costs vary by payer, provider type and state. Credentialing frequently takes several months to complete.
- Denial rates vary significantly by payer, specialty and practice. The adjustable defaults are illustrative planning assumptions, not guaranteed national averages.
- Contractual adjustments (the difference between gross charges and contracted allowable) are not classified as preventable lost revenue in this calculator.
- CareVixis pricing and inclusions shown here are controlled through the page configuration object and reflect current published rates.
