Revenue management for plastic surgery

Two Words Decide Whether You Are Paid. Does Your Record Establish Them?

The exclusion is short and the exceptions are shorter. Cosmetic surgery and related services are excluded, except as required for the prompt repair of accidental injury, or to improve the functioning of a malformed body member. Everything a plastic surgery practice bills to a medical plan has to land inside one of those two exceptions, and the words carrying them are prompt and functioning. Neither is established by the operative note alone.

The Exclusion Is Short.

Cosmetic surgery and related services are excluded. Then two exceptions, in one sentence.

There is no third category.

And the Exceptions Are Narrow.

Prompt repair of accidental injury. Or improving the functioning of a malformed body member.

Timing and cause in one, function in the other.

Function Is Not the Same as Deformity.

A record can establish the malformation perfectly and never say what the patient cannot do.

That record has evidenced the wrong half of the test.

And It Was Written at the Consultation.

Functional history, cause, dates, photographs taken for a clinical reason.

None of it is created by the operative note, and none of it can be added later.

That Is the Work.

CareVixis manages authorisations, claims and appeals around your existing systems, and says when a record will not support one.

Nothing clinical changes.

You cannot code around a rule written about purpose.

The detail

The detail, if you want it

What the exclusions say about cosmetic surgery, consultations and dental structures, where plastic surgery practices lose money, and the questions worth asking about your own revenue.

When is plastic surgery covered rather than excluded?

When it falls within one of two narrow exceptions. The regulation excludes “cosmetic surgery and related services, except as required for the prompt repair of accidental injury or to improve the functioning of a malformed body member”. There is no third category and no partial credit.

The exceptions do different work. One is about timing and cause: prompt repair, of an accidental injury. The other is about function: improving how a malformed body member works, rather than how it looks. A practice that documents the deformity without documenting the functional consequence has evidenced the wrong half of the test.

What the rules actually say

What the Regulation Actually Says

['Quotations from 42 CFR Part 411, cited below. Three separate exclusions reach plastic surgery, and reconstructive work can run into more than one of them at once.', 'It is worth noticing that these are statutory-style exclusions written about purpose and effect. They are not code descriptions, which is why choosing a different code never resolves them.']

Published rules and coding families for this specialty, cited below. CareVixis states them as the source publishes them and does not paraphrase a payer policy into a promise.
Where it bitesWhat the published rule says
The exclusion“Cosmetic surgery and related services, except as required for the prompt repair of accidental injury or to improve the functioning of a malformed body member.”
The first exceptionPrompt repair of accidental injury. Both the cause of the defect and the timing of the repair are part of the test.
The second exceptionTo improve the functioning of a malformed body member. Function is the operative idea, not appearance and not the presence of the malformation.
Related services tooThe exclusion reaches “related services”, not only the surgery, so the work around an excluded procedure follows it.
Consultations with no medical purposeSeparately excluded are examinations “performed for a purpose other than treatment or diagnosis of a specific illness, symptoms, complaint, or injury”.
Where dental structures are involvedAlso excluded are “dental services in connection with the care, treatment, filling, removal, or replacement of teeth, or structures directly supporting the teeth”.
And its exceptionThat dental exclusion does not apply to “inpatient hospital services in connection with such dental procedures when hospitalization is required because of” the individual’s underlying medical condition and clinical status, or the severity of the dental procedures.

CareVixis states the regulation as it is written and does not advise on how it applies to a particular case. Commercial plans apply their own medical policies, which are often stricter and always separate.

The gap

The Evidence Is Gathered Before the Operating Room

Whether a repair was prompt is a fact about dates. Whether a procedure improves functioning is a fact about what the patient could not do before and can do after. Both are established in the consultation, the referral, the photographs and the functional history, and none of them is created by the operative note.

So the record that wins an appeal is assembled months before anyone appeals anything. Practices that treat pre-authorisation as an administrative hurdle tend to submit the minimum that gets an approval, and then discover that the minimum is also all they have when a payer revisits the claim.

There is a second exposure peculiar to this specialty: the same patient frequently receives covered and non-covered work in one operative session. Separating them cleanly, in the consent, the record and the claim, is the difference between a defensible practice and a difficult conversation.

What supports the exceptions

  • A documented functional limitation.
  • The cause and date of an injury.
  • Photographs taken for a clinical purpose.
  • A referral describing a problem, not a wish.

What does not

  • A diagnosis naming the deformity alone.
  • An operative note describing technique.
  • A payer approval with no reasoning retained.
  • Patient dissatisfaction with appearance.
Where this specialty loses money

Where a Plastic Surgery Practice Loses Money

Not every practice sees all of these. They are what CareVixis expects to look at first on a plastic surgery account.

Deformity documented, function not

The exception is about improving how something works. A record that establishes the malformation without establishing the functional consequence has met half the test.

Prompt, undefined and unexamined

The accidental injury exception carries a timing word. A repair performed long after the injury invites the question, and the answer needs to be in the record.

Mixed sessions billed as one thing

Covered and non-covered work in a single operative session has to be separated in the consent, the record and the claim. Practices often separate it in only one of the three.

Authorisation obtained, reasoning discarded

An approval is not a defence on its own. What was submitted to obtain it is the evidence, and it needs to still exist when the claim is reviewed.

Related services following the exclusion

The exclusion reaches related services. Anaesthesia, facility and follow-up around an excluded procedure are not rescued by being billed separately.

Reconstruction touching dental structures

Work involving teeth or the structures directly supporting them meets a separate exclusion, whose exception is specifically about inpatient hospital services where hospitalisation is required.

CareVixis publishes no denial rate, recovery rate or resolution time for plastic surgery or any other specialty, does not advise on coverage for particular cases, and does not suggest any practice experiences all of this.

Conceptual workflow

Where This Fits Around What You Already Run

Where a plastic surgery claim is decided, and where CareVixis joins it. Almost all of it happens before the procedure.

  1. ConsultationWhere the functional history, the cause of the defect and the clinical photographs are captured, or are not.
  2. AuthorisationSubmitted with the reasoning that establishes an exception, and retained afterwards as the evidence it is.
  3. The sessionCovered and non-covered work separated in consent and record where both are performed.
  4. Claim and related servicesThe procedure and everything around it, billed consistently with which exception applies.
  5. VixisIQ™CareVixis revenue intelligence reads the resulting remittance information, where appropriate data can be obtained and normalized, and looks for patterns clustered on procedure and payer policy rather than on single claims.
  6. VixisTrace™Where a claim was denied, rejected or underpaid, CareVixis claim root cause analysis works eight stages: Detect, Investigate, Identify, Correct, Prevent, Resubmit, Verify, Monitor.
  7. The CareVixis teamPeople assemble the appeal from the record that exists, and say plainly when the record will not support one. Denials are worked the day they arrive; submitting the corrected claim or appeal the same day is the CareVixis standard.
A framework you can use on anyone

Questions Worth Asking About Your Own Revenue

Put these to CareVixis too. In this specialty an honest answer about a weak record is worth more than an optimistic appeal.

  1. Does our consultation record functional limitation?

Ask to read three. If they describe appearance rather than what the patient cannot do, the exception is not evidenced.

  2. Do we keep what we submitted for authorisation?

Ask where it is stored. The approval is not the evidence; the submission is.

  3. How do we handle mixed sessions?

Ask how covered and non-covered work is separated in consent, record and claim, and whether all three agree.

  4. Who checks payer medical policy before scheduling?

Commercial policies are stricter than the regulation and differ from each other. Ask when they are read.

  5. What do we do about delayed repairs?

Ask how the record addresses timing where a repair follows an injury by a long interval.

  6. Are related services billed consistently?

Ask whether anaesthesia, facility and follow-up follow the same coverage conclusion as the procedure.

  7. Will you tell us when an appeal is not worth making?

Ask directly. A partner who appeals everything is not exercising judgement on your behalf.

  8. How is patient responsibility quoted?

Ask what patients are told before non-covered work, and what happens when the estimate turns out wrong.

  9. Where are the people working our claims?

Ask whether you can speak to them directly. CareVixis is 100% US based and does not outsource.

Using Plastic surgery? Keep It.

Keep the practice management, imaging and scheduling systems you already run. CareVixis manages the revenue cycle around them and does not require a software change to begin.

Keeping your system is a supported outcome, not a fallback. If a practice later wants to move to the CareVixis platform, that move may be offered at no additional migration charge, and everything the patient sees still carries the practice's own name and branding.

Keep your systems. Build the record the exception actually needs.

Straight answers

Questions Plastic Surgery Practices Ask

Is cosmetic surgery ever covered by Medicare?

The regulation excludes cosmetic surgery and related services except as required for the prompt repair of accidental injury or to improve the functioning of a malformed body member. Those two exceptions are the whole of it.

What does improving functioning mean?

The exception is written about function rather than appearance. A record that establishes a malformation without establishing what the patient cannot do because of it has not addressed the part of the test the exception turns on.

Does the exclusion cover more than the surgery?

Yes. It applies to cosmetic surgery and related services, so work around an excluded procedure is not rescued by being billed separately.

What about reconstruction involving teeth?

A separate exclusion covers dental services in connection with the care, treatment, filling, removal or replacement of teeth or structures directly supporting them. Its exception is for inpatient hospital services where hospitalisation is required because of the individual's underlying medical condition and clinical status, or the severity of the dental procedures.

Does CareVixis advise on whether a case is covered?

No. CareVixis states what the published rules say, manages the claims and appeals, and will tell you plainly when a record will not support an appeal. Coverage judgements for particular patients are for the practice and its advisers.

What is VixisIQ™?

VixisIQ™ is the CareVixis revenue intelligence technology. It analyses revenue cycle and remittance data to surface payment patterns, adjustments, denial and rejection patterns, payer behaviour and accounts receivable trends. What it can analyse for a particular practice depends on the data that practice is able to make available.

What is VixisTrace™?

VixisTrace™ is the CareVixis claim root cause analysis technology, covering rejected, denied, underpaid and otherwise problematic claims across eight stages. The objective is continuous improvement: identify preventable patterns, address their causes, and reduce unnecessary repeat failures.

Can CareVixis work aged plastic surgery balances?

Yes, as a separate arrangement charged on what is actually recovered rather than on the balance handed over. Terms are on the pricing page.
