For community behavioral health organisations using QualifactsYour Rate Is Set. Does That Mean Your Revenue Is?
A bundled or prospective rate removes some risk and relocates the rest. The loss stops looking like denials and starts looking like encounters nobody captured, clients whose eligibility lapsed, and the service lines that are still plain fee-for-service. Keep Qualifacts. Find out where yours went.

Your Rate Is Settled. That Feels Like Safety.
A bundled or prospective rate takes the per-claim anxiety out of the month, and for an organisation delivering community services that is genuinely steadying.
A Fixed Rate Still Rests on Encounters.
The rate is paid against services that were delivered, documented and reported correctly. Change any of those three and the rate does not protect you.
So the Exposure Moves Upstream.
An encounter that was delivered but never documented properly, or documented but never reported, does not fail loudly anywhere.
It simply does not count, and it does not count quietly.
We Check the Encounters Under the Rate.
CareVixis reconciles what was delivered against what was documented and reported, and reads the remittances alongside it, because under a bundled rate that is where the revenue is actually decided.
Keep Qualifacts.
Qualifacts is built for the work you do and there is no case here for replacing it. The platform stays and the revenue reconciliation sits beside it.
Keep Qualifacts. Make sure the encounters underneath your rate are real.
Everything below is the evidence for what you just read.
What Qualifacts publishes about its own platform, why a settled rate does not settle the revenue, a ten-question framework, and the source for every fact on this page.
Does a bundled rate mean revenue cycle work matters less?
No. It changes which mistakes cost you. Under fee-for-service, an uncaptured encounter is a missing claim and somebody eventually notices. Under a rate tied to encounters, clients or days, an uncaptured encounter can quietly weaken the basis of the rate itself.
Meanwhile the parts of the organisation that never moved to a bundled arrangement are still billing the old way, still being denied the old way, and are often watched less closely precisely because leadership attention followed the larger programme.
Three Platforms, One Vendor, Different Questions
Which Qualifacts platform you are on is not a detail. It shapes what data exists and how it can be read. Everything below is as Qualifacts publishes it.[1]
| What it does | How Qualifacts describes it |
|---|---|
| Three EHR products | Qualifacts publishes Credible, CareLogic and InSync as its EHR solutions for behavioral health organisations. |
| Credible | Described as supporting the largest enterprise agencies, including organisations with CCBHC operations or multiple locations, programmes and specialties. |
| CareLogic | Described as suited to enterprise behavioral health and human services organisations, including multi-location and multi-state entities and agencies providing value-based care. |
| Core functions | Scheduling, treatment plans, assessment tools, billing solutions and state reporting. |
| CCBHC focus | Qualifacts states that its customer base includes a substantial share of the nation's Certified Community Behavioral Health Clinics, and positions the platforms around adherence to CCBHC programmes and value-based reimbursement. |
| Scale | Qualifacts states it serves thousands of organisations across all fifty states, non-profit and for-profit. |
State reporting sitting in the same list as billing is the tell. In this sector they are two descriptions of the same work, and when they disagree it is usually the revenue that is wrong rather than the report.
A Rate Does Not Audit Itself
The appeal of a prospective rate is predictability, and it delivers that. What it also does is weaken the feedback loop: when payment does not vary claim by claim, the claim-by-claim signal that something is wrong gets quieter.
Most agencies under a bundled arrangement still run fee-for-service lines alongside it. Different programmes, different payers, sometimes different sites. Those lines behave exactly like ordinary billing and fail exactly like ordinary billing, while receiving a fraction of the attention.
So the question is not whether your rate is right. It is whether anybody is still reading the parts of your revenue that can go wrong the ordinary way, and whether the encounters underpinning the rate are all actually there.
Six Places a Community Organisation Loses Revenue
These are the failures that survive a well run billing department, because most of them never reach the billing department at all.
Encounters that were never captured
Care delivered, documented somewhere, and never recorded as a countable encounter. Under a rate tied to encounters, that is the foundation moving.
The fee-for-service remainder
Programmes outside the bundled arrangement, still denied for ordinary reasons, watched less because they are the smaller number.
Eligibility lapses between visits
Populations served by community organisations move on and off coverage. Redetermination cycles turn into months of unbillable care if nobody is checking between services.
Site-level variation
One clinic does intake differently from another. At organisation level it averages out into a number nobody can act on.
Staff turnover in the billing chain
When the person who understood a payer's quirk leaves, the quirk stays. Institutional knowledge is a revenue asset and it is rarely documented.
Reports and claims that disagree
The state sees one count of service, the payer sees another. The gap is a reconciliation nobody owns until somebody audits it.
None of these is fixed by working denials harder. They are found by comparing what was delivered against what was captured, and what was captured against what was paid, per site and per programme.
Where This Fits Around What You Already Run
An illustration of the general flow for an organisation keeping Qualifacts, not a description of any specific configuration.
- Service delivered and documentedAcross programmes and sites, in Credible, CareLogic or InSync.
- Encounter captureThe step that matters most under a bundled rate, and the one with the least feedback when it fails.
- Claims and bundled billingFee-for-service lines are adjudicated claim by claim; bundled lines are paid on the agreed basis.
- Remittance and reportingPayments, denials and adjustments on one side, state reporting on the other.
- VixisIQ™Where appropriate data can be obtained and normalized, encounters, claims and payments are compared per programme, per site and over time.
- VixisTrace™A failed claim on a fee-for-service line is investigated for a root cause rather than reworked.
- One number leadership can defendWhat was delivered, what was captured, what was paid, and where the three part company.
How Should a Community Organisation Evaluate a Revenue Partner?
Ten questions for whoever handles your revenue today, and for CareVixis. The first three separate people who have worked in this sector from people who have not.
-
Do they understand bundled and prospective arrangements?
Ask what they would watch differently under a bundled rate. If the answer is the same as for a private practice, they have not worked here.
-
Can they reconcile encounters against claims?
Not denials against claims. Delivered care against captured encounters, which is the harder and more valuable comparison.
-
Will they watch the fee-for-service remainder?
Ask specifically who is responsible for the programmes outside your main arrangement.
-
Can they report by site and by programme?
Organisation-level totals are exactly the wrong resolution for a multi-site agency.
-
How do they handle eligibility churn?
Ask what happens between a client losing coverage and their next appointment.
-
Do they reconcile state reporting against billing?
Ask who compares the two counts and what happens when they differ.
-
What happens the third time the same denial arrives?
Ask for a root cause they found, what changed, and how they knew it worked.
-
Will they work inside your Qualifacts platform?
Ask what they need from Credible, CareLogic or InSync specifically. CareVixis treats keeping your platform as a supported outcome.
-
What will the board see, and how often?
Then the harder version: what would have told you one site stopped capturing encounters in August?
-
Is the work performed in the United States?
Ask about subcontractors as well as staff. CareVixis is 100% US based and does not outsource.
Using Qualifacts? Keep It.
Your programmes, your state reporting and your compliance posture are configured in it, and for a multi-site organisation that configuration represents years of work. A migration would answer none of the questions on this page.
Keeping your system is a supported outcome, not a fallback. If you later want to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis.
Keep Qualifacts. Make sure the encounters underneath your rate are real.
Questions Community Organisations Ask
Does a bundled rate mean revenue cycle work matters less?
No. It changes which mistakes cost you. Under fee-for-service an uncaptured encounter is a missing claim that somebody eventually notices. Under a rate tied to encounters, clients or days, an uncaptured encounter can quietly weaken the basis of the rate itself.
Do I have to leave Qualifacts to work with CareVixis?
No. Keeping Qualifacts is a supported outcome, not a fallback. CareVixis claims no API integration, certification or partnership with Qualifacts, and what is agreed before any commitment is which revenue-cycle data can be obtained and how.
Does it matter whether we are on Credible, CareLogic or InSync?
It matters for what data exists and how it comes out, which is a practical question rather than a judgement about the products. It is the first thing worth establishing, because it decides what any analysis can honestly say.
Most of our revenue is not fee-for-service. Is this still relevant?
Usually more so. The parts of an organisation that still bill claim by claim tend to get the least attention once a larger bundled programme exists, and they fail in the ordinary ways while nobody is watching.
Does CareVixis advise on CCBHC compliance or cost reporting?
No. Programme certification, cost reporting and state compliance are specialist functions that sit with your finance and compliance leadership. CareVixis works on the revenue cycle underneath: what was delivered, what was captured, what was paid, and why those disagree.
