For ambulatory practices using athenahealthYour EHR Vendor Is Also Your Billing Company. Who Checks the Work?
That is not an accusation, and it is not unique to one vendor. It is a structural point: when software, billing and the reporting on billing all come from one place, there is nobody outside the arrangement reading the result. Keep athenaOne. Get an independent read.

Buying Both From One Company Makes Sense.
athenaOne gives you the record and the billing service together, with nobody to blame anybody else when something goes wrong.
Practices choose that on purpose, and it is a reasonable thing to want.
It Also Means One Company Marks Its Own Work.
The party doing your billing is the party reporting on your billing. That is not dishonesty. It is just a loop with nobody outside it.
Every number you see about the work came from the people doing the work.
What Never Gets Asked.
Whether this month's denials share a cause. Whether the appeal that was filed was ever actually paid. Whether a payer quietly started paying one of your codes differently.
A report answers the question it was built to answer, and no more.
A Second Reader Asks Different Billing Questions.
CareVixis reads your athenahealth remittance data from outside the billing service, on a schedule, grouping denials by cause rather than working them one claim at a time.
Different vantage point, different questions about the same revenue, and that is the entire value.
Keep athenaOne.
This is not an argument to unpick a stack that works. Keeping athenaOne is a supported outcome, not a fallback, and the billing service carries on exactly as it does now.
Keep athenaOne. Get a second set of eyes on the numbers.
Everything below is the evidence for what you just read.
What athenahealth publishes about athenaOne, what an in-house report cannot be expected to ask, a ten-question framework you can put to athenahealth and to CareVixis alike, and the source for every fact on this page.
Is this page saying athenahealth does billing badly?
No, and it would be dishonest to imply it. CareVixis has no visibility into how any arrangement performs for your practice, and athenahealth has been doing revenue cycle work at scale for a long time.
The point is narrower and it applies to every bundled arrangement, including one with CareVixis: the party doing the work should not be the only party measuring it. If your billing is performed by your software vendor, then the dashboard telling you how billing is going is produced by the team whose performance it describes. That is worth having a second opinion on, in the same way you would for anything else material to the practice.
athenaOne Is a Large, Capable Arrangement
Understanding what you have bought is the first step to knowing what is missing. Everything below is athenahealth's own published description.[1]
| What it does | How athenahealth describes it |
|---|---|
| What athenaOne is | Described as a solution for ambulatory practices that includes EHR, practice management and patient engagement capabilities and services. |
| Services, not only software | The word services is athenahealth's own. The arrangement includes work performed on the practice's behalf, not just a product licence. |
| Documentation | Tailorable documentation and more complete records are described as part of the EHR component. |
| Positioning | athenahealth positions the platform around reducing burdensome work and improving organisational performance. |
| Scope | Ambulatory practice performance across clinical, administrative and patient-facing functions. |
A practice that has bought software and services together has genuinely simplified its vendor management. What it has also done is concentrate several roles in one relationship, and concentration is worth looking at deliberately rather than by accident.
Nobody Grades Their Own Homework Well, Including Us
Every billing arrangement produces reporting. Clean claim rate, days in accounts receivable, collections against expectation. Those are real measures and they are usually accurate.
What in-house reporting rarely answers is the question it is least comfortable with: is this as good as it should be? A clean claim rate says claims left cleanly. It does not say whether the payment that came back matched your contract, whether a denial was worked or written off, or whether a recurring cause was ever investigated rather than reworked.
An independent read is not an audit or an accusation. It is a second set of numbers, derived from your remittance data rather than from the workflow, answering questions the workflow does not ask about itself.
What the arrangement reports
- Claims submitted and accepted.
- Days in accounts receivable.
- Collections against expectation.
- Denials worked and closed.
What an independent read asks
- Did the payment match the contracted rate?
- Was that denial resolved, or written off?
- Has the same cause recurred all year?
- Which payer changed behaviour, and when?
Six Questions Worth Asking of Any Billing Arrangement
These apply whoever does your billing: an outside company, an in-house team, your software vendor, or CareVixis.
Underpayment against contract
A claim that pays is a claim that closes. Whether it paid the contracted amount is a separate check, and it is the one most commonly skipped.
Write-off authority
Ask who can decide a balance is not worth pursuing, at what threshold, and whether you see the total before it happens.
Denial resolved or denial closed
A queue empties either way. The distinction between recovered and abandoned is not always visible in the reporting.
Recurring causes
Working the same denial competently every month is good service and bad outcome. Ask what happens on the third occurrence.
Specialty and contract nuance
Rules engines built from broad network data are powerful and general. Your contracts and your specialty's quirks are specific.
Who tells you when it slips
The most useful question in this whole page: if performance dropped in March, what would have told you, and who would have sent it?
A good billing partner welcomes these questions, because a partner performing well has nothing to lose from a second opinion. Ask CareVixis the same six.
Where This Fits Around What You Already Run
An illustration of the general flow for a practice keeping athenahealth, not a description of any specific configuration.
- athenaOneThe encounter is documented, coded and billed within the arrangement you already have.
- Clearinghouse and payerThe claim is transmitted and adjudicated, or rejected before adjudication.
- Remittance returnsPayment, adjustment, denial and patient responsibility, with the payer's reason and remark codes.
- VixisIQ™, reading the same data independentlyWhere appropriate data can be obtained and normalized, payments are compared against expectation and denial patterns counted across payers and time.
- VixisTrace™A recurring denial is investigated for a root cause, independent of whoever worked the claim.
- A second opinion you can act onEither a confirmation that the arrangement is performing, which is worth knowing, or a specific list of what is not.
How Should You Evaluate Any Billing Arrangement, Including This One?
Nine questions. Put them to your current arrangement and to CareVixis, and compare the answers rather than the brochures.
-
Who measures the billing performance?
If the answer is the team doing the billing, that is normal and it is still worth a second read.
-
Are payments checked against contracted rates?
Ask how a quiet reduction in what a payer pays for a routine code would be detected.
-
What is the write-off policy, in writing?
Threshold, authority and reporting. Ask to see last quarter's total.
-
What happens the third time the same denial arrives?
Ask for a specific root cause found in the last year, what changed, and how they knew it worked.
-
Can you get your own data out?
A practical question with a long-term answer. If analysis requires the vendor's cooperation, you have one supplier for both the work and the evidence.
-
Is the reporting comparable to anything external?
Ask which figures are defined the way the rest of the industry defines them.
-
Who owns credentialing and enrollment dates?
In a multi-provider practice this remains a common cause of denials that present as something else.
-
What would have told you performance slipped?
The best single question. Ask for the mechanism, not the intention.
-
Is the work performed in the United States?
Ask about subcontractors as well as staff. CareVixis is 100% US based and does not outsource.
Using athenahealth? Keep It.
Your clinical workflows, your scheduling and your billing all run through it, and unpicking that is a large project with real risk. Nothing on this page suggests you should.
Keeping your system is a supported outcome, not a fallback. If you later want to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis.

Questions athenahealth Practices Ask
Is this page saying athenahealth does billing badly?
No, and it would be dishonest to imply it. CareVixis has no visibility into how any arrangement performs for your practice. The point is narrower and applies to every bundled arrangement, including one with CareVixis: the party doing the work should not be the only party measuring it.
Do I have to leave athenahealth to work with CareVixis?
No. Keeping athenahealth is a supported outcome, not a fallback. CareVixis claims no API integration, certification or partnership with athenahealth, and what is agreed before any commitment is which revenue-cycle data can be obtained and how.
Can CareVixis just review our performance without taking over billing?
That is a reasonable way to start, and it is the honest version of what this page offers. An independent read of your remittance data either confirms the arrangement is performing, which is worth knowing, or produces a specific list of what is not.
Our clean claim rate is high. Does that settle it?
It settles one thing: claims are leaving correctly. It says nothing about whether payments matched your contracts, whether denials were recovered or written off, or whether a recurring cause has been investigated rather than reworked each month.
Would CareVixis be subject to the same question?
Yes, and it should be. If CareVixis manages your revenue cycle then CareVixis is reporting on its own work, which is exactly the structure this page is about. The answer is the same in both directions: ask for the mechanism that would tell you when performance slips.
