Your Scrubber Catches What It Knows. What About the Rest?

Claim scrubbing is prevention, and prevention works on the errors somebody already encoded as a rule. Every denial that reaches you is, by definition, one the rules did not anticipate. Keep AdvancedMD. Find out what got through, and why it keeps getting through.

Your Scrubber Is Doing Its Job.

AdvancedMD checks your claims before they go out and catches the things a rule can state: a missing field, a bad format, a code that does not belong with another one.

That work is real, and for most claims it is enough.

A Rule Only Knows What It Was Told.

A scrubber cannot know that a payer changed its policy in March, or that one plan started wanting an authorization it never wanted before.

Nobody wrote a rule for it, because nobody knew yet.

So the Denial Arrives Anyway.

The claim was clean by every rule in the system and the payer still said no. Nothing in the software is responsible for asking why it disagreed.

The claim gets worked. The reason goes unrecorded.

Somebody Has to Ask Why the Denial Happened.

CareVixis reads the AdvancedMD denials that got through the scrubber, records a denial reason for each one, and counts them.

Then a scrubbing rule can be updated, a billing step added, or the payer's new requirement simply met.

Keep AdvancedMD.

Nothing here argues for replacing AdvancedMD. The scrubber stays, your billing workflow stays, and the only change is that the denials it could not prevent finally get explained.

Keep AdvancedMD. Explain the denials it could not prevent.

The detail

Everything below is the evidence for what you just read.

What AdvancedMD publishes about its own platform, where denials survive a clean scrub, a ten-question framework you can put to any billing company, and the source for every fact on this page. If you have what you came for, you can stop here.

If claims are scrubbed before submission, why are they still denied?

Because a scrubber encodes known rules, and a denial is what happens outside them. Scrubbing catches missing identifiers, invalid code combinations, format problems and the payer rules the vendor has already learned. It is genuinely valuable and it raises the floor.

What it cannot catch is a payer changing a policy last month, a clinician's enrollment lapsing, an authorization that did not cover what was delivered, or a contract paying less than it should. None of those is a malformed claim. They are perfectly valid claims that the payer declines, or pays short.

Credit where it is due

AdvancedMD Puts a Lot Between You and a Bad Claim

This is a well equipped billing stack, and the argument on this page is about the boundary of what any such stack can do. Everything below is as AdvancedMD publishes it.[1]

AdvancedMD capability as AdvancedMD publishes it. CareVixis has not independently tested these features and states them as the vendor's own descriptions.
What it doesHow AdvancedMD describes it
Integrated clearinghouseAn integrated clearinghouse that automates claim submission and remittance processing, removing manual file upload and download.
Payer reachAdvancedMD states its clearinghouse connects to roughly eighteen hundred carriers.
ScrubbingAn electronic claims scrubbing engine that checks claims against compliance, code edits and payer rules before submission.
Pre-visit checksCopays, deductibles and coverage described as automatically verified prior to each visit.
WorklistsDashboards and worklists showing outstanding claims, aging, denials and follow-up tasks.
Unified platformCharge information flowing automatically from scheduling and clinical documentation into billing.
Services optionA revenue cycle management service covering billing management, claims submission, payment posting, denial management and dedicated account management.

Notice what is on that list and what is not. Submission, validation, posting and worklists are all there. An explanation of why a specific denial happened is a different kind of work, and no scrubber produces one.

The gap

Prevention and Explanation Are Different Jobs

A worklist tells you a claim was denied and lets you act on it. It does not tell you why the denial occurred in your practice, and it cannot, because the reason code describes the payer's decision rather than your process.

So the denials that survive a good scrubber are the interesting ones. They are, almost by definition, the ones caused by something outside the rules: a policy change, an enrollment date, an authorization mismatch, a contract discrepancy, a payer behaving differently this quarter than last.

Working them clears the queue. Explaining them is what stops the queue refilling with the same thing next month.

What a scrubber prevents

  • Missing or malformed identifiers.
  • Invalid code combinations.
  • Known payer formatting rules.
  • Obvious eligibility gaps before the visit.

What it cannot see

  • A payer policy that changed last month.
  • A clinician enrolled from next Tuesday.
  • An authorization that did not cover this.
  • A payment below the contracted rate.
Why this practice type, specifically

Six Denials a Clean Claim Still Gets

Every one of these arrives on a claim that passed validation, which is what makes them frustrating and easy to misread.

The payer changed its policy

Nothing about your claim changed. The rule on the other side did, and you find out one denial at a time.

Enrollment effective dates

A valid claim for a clinician who was not yet enrolled with that plan. The reason code rarely says so plainly.

Authorization mismatch

An authorization exists but does not match the units, dates or site delivered. The claim is well formed and still refused.

Underpayment against contract

The worst case, because the claim pays and closes. Nothing in a denial worklist will ever show it to you.

Coordination of benefits

Primary and secondary in the wrong order, or other coverage the payer knows about and you do not.

Medical necessity under that plan

A judgement against the payer's policy. No pre-submission check can settle it, because it is not a data problem.

Five of those six will pass any scrubber ever built, and the sixth pays you less while looking like a success. That is the case for investigating denials rather than only working them.

Conceptual workflow

Where This Fits Around What You Already Run

An illustration of the general flow for a practice keeping AdvancedMD, not a description of any specific configuration.

  1. AdvancedMDThe charge flows from scheduling and documentation, and the claim is scrubbed against known rules.
  2. Integrated clearinghouse and payerThe claim is transmitted and adjudicated, or rejected before adjudication.
  3. Remittance returnsPayment, adjustment, denial and patient responsibility, with the payer's reason and remark codes.
  4. The worklistWhat got through the rules lands here to be worked. This is where most practices stop.
  5. VixisIQ™Where appropriate data can be obtained and normalized, what got through is counted by payer, provider and code over time.
  6. VixisTrace™One survivor is investigated properly, which is how a new payer policy is found before the tenth claim hits it.
  7. A rule worth adding, or a process worth fixingSome findings become a scrubbing rule. Most become a change somewhere earlier than the claim.
A framework you can use on anyone

What Should an AdvancedMD Practice Ask a Billing Company?

Nine questions for whoever bills for you now, including any service you already buy, and for CareVixis.

  1. What do they do with the denials that pass scrubbing?

The central question on this page. Working them is table stakes; explaining them is not.

  2. Are payments checked against contracted rates?

Ask how a quiet underpayment on a routine code would ever be noticed.

  3. How would they spot a payer policy change?

Ask for the mechanism. A payer that tightens a rule will usually tell you only through denials.

  4. Who owns enrollment effective dates?

Ask what happens to claims for a clinician who started before their date, and who decides to hold them.

  5. Can they report denials by provider and by code?

The grouping is where a pattern stops being anecdote.

  6. What happens the third time the same denial arrives?

Ask for a root cause they found, what changed, and how they knew it worked.

  7. Will they work inside AdvancedMD?

Ask what they need from it and what changes if you keep it. CareVixis treats keeping AdvancedMD as a supported outcome.

  8. What will you see without asking?

Then the harder version: what would have told you one payer started denying a routine code in June?

  9. Is the work performed in the United States?

Ask about subcontractors as well as staff. CareVixis is 100% US based and does not outsource.

Using AdvancedMD? Keep It.

Your scheduling, documentation, clearinghouse and billing are already connected in it, and that integration is doing real work every day. A migration would answer none of the questions on this page.

Keeping your system is a supported outcome, not a fallback. If you later want to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis.

Keep AdvancedMD. Explain the denials it could not prevent.

Straight answers

Questions AdvancedMD Practices Ask

If claims are scrubbed before submission, why are they still denied?

Because a scrubber encodes known rules, and a denial is what happens outside them. It cannot catch a payer changing a policy last month, a clinician's enrollment lapsing, an authorization that did not cover what was delivered, or a contract paying less than it should. Those are valid claims that the payer declines, or pays short.

Do I have to leave AdvancedMD to work with CareVixis?

No. Keeping AdvancedMD is a supported outcome, not a fallback. CareVixis claims no API integration, certification or partnership with AdvancedMD, and what is agreed before any commitment is which revenue-cycle data can be obtained and how.

We already use the AdvancedMD revenue cycle service. Is this instead of that?

It can be, and it does not have to be. If the service is working, keep it, and use the framework above to establish whether it is. Where CareVixis differs is that root cause analysis and pattern analysis are the centre of the work rather than an extra.

Would better scrubbing rules fix this?

Some of it, and that is worth doing. But a rule can only encode something already understood, so rules always trail the payer. The value of investigating a denial is that it tells you what rule to add, or more often that the fix belongs somewhere earlier than the claim.

What about claims that paid?

They are the blind spot. A denial worklist never contains a claim that paid less than your contract says it should, so nothing in the ordinary workflow will ever raise it. Checking payment against expectation is a separate job that has to be asked for.
