For behavioral health and addiction treatment programmes using BestNotesYour Records Satisfy Your Surveyor. Do They Satisfy Your Payer?
Accreditation and reimbursement are two different tests of the same chart, and passing one says nothing about the other. Keep BestNotes. Find out which payer is asking for something your survey standards never did.

Your Charts Pass Inspection.
BestNotes was built for programmes that get surveyed, and your records hold up when somebody comes to look at them.
That is not a small thing and it is genuinely hard to do.
A Surveyor and a Payer Want Different Things.
A surveyor asks whether the care was documented properly. A payer asks whether this specific element, on this specific date, in this specific form, justifies this specific code.
Both are reasonable. They are not the same question.
So a Good Chart Still Loses the Claim.
The note is thorough, the care was delivered, and the claim comes back anyway because one element the payer wanted was recorded somewhere it does not look.
Nothing about the clinical record was wrong.
We Find Out What the Payer Is Asking For.
CareVixis reads the denials against the documentation and records which element each payer actually wanted, so the billing gap gets named instead of guessed at.
Keep BestNotes.
The programme keeps BestNotes, your staff keep their workflow, and the clinical record does not change to chase a payer. Only the denial work moves.
Keep BestNotes. Find out what the payer is asking for.
Everything below is the evidence for what you just read.
What BestNotes publishes about its own platform, the six gaps between a good chart and a paid claim, a ten-question framework you can put to any billing company, and the source for every fact on this page. If you have what you came for, you can stop here.
Our documentation passes survey. Why are claims still denied?
Because a surveyor and a payer are asking different questions. A surveyor asks whether your programme does what it says it does, consistently, to a recognised standard. A payer asks whether this service, for this member, on this day, met its own coverage criteria.
Both are legitimate. They are simply not the same test, and a chart can pass the first while failing the second. Practices that discover this usually respond by documenting more, which is expensive and often aimed at the wrong thing.
BestNotes Starts Before the Admission
Most EHRs begin at the encounter. BestNotes begins at the enquiry, which for a programme is where revenue is won or lost. Everything below is BestNotes' own published description.[1]
| What it does | How BestNotes describes it |
|---|---|
| Who it is for | An EHR "for therapy, behavioral health, and addiction treatment", supporting "solo practices to enterprise behavioral health organizations". |
| Intake pipeline | A "secure online client intake application" linked to your website, with a "prescreen application that auto-populates into intake and clinical documentation". |
| Insurance at the front door | "Capture insurance information prior to admission." |
| Admission control | "Admission criteria checklists", staff notification of new admissions, and capture of "staff signature for admission approval". |
| Accreditation-aware documentation | Documentation "that takes into account Joint Commission, CARF and common National/State/County standards that's automatically updated for you". |
| Documentation tooling | "AutoNotes" with point and click documentation, and auto-population to eliminate double entry. |
Capturing insurance before admission is the single most valuable revenue feature on that list, and plenty of programmes still do not do it. Having the field is not the same as someone acting on what it says.
Six Gaps Between a Good Chart and a Paid Claim
These are the failures that show up in programmes with strong documentation, which is what makes them frustrating: the obvious explanation has already been ruled out.
The standard the record was built for
Accreditation language and payer criteria overlap but do not match. The elements one asks for are not always the elements the other looks for.
Insurance captured, but not acted on
The field is filled at intake. Whether anyone verified the benefit, and whether anyone compared it to the programme being offered, is a different question.
Admission criteria met clinically, not contractually
Your checklist says admit. The payer's criteria for that level of care may say something else, and it is the payer's list that determines payment.
The prescreen never reaches the claim
Information gathered before admission tends to live in intake. When the claim is built from the clinical record alone, useful detail is simply not there.
Enterprise scale hides the pattern
Across several programmes and locations, a problem that is obvious at one site is a rounding error in the group total until someone looks per site.
It was never a documentation problem
Eligibility, enrollment, a carve-out or a coding mismatch, arriving under a reason code that points everyone at the chart. The record gets improved; the denial keeps coming.
Documentation is the most expensive thing to change in a programme, because the cost lands on clinical staff. Establishing which of the six is actually happening, before anyone is asked to write differently, is the whole point of investigating a denial rather than reworking it.
Where This Fits Around What You Already Run
An illustration of the general flow for a programme keeping BestNotes, not a description of any specific configuration.
- Enquiry, prescreen and intakeInsurance is captured before admission and admission criteria are checked.
- BestNotesCare is delivered and documented against your accreditation standards.
- Claims, clearinghouse and payerServices are billed and adjudicated, in whole or in part.
- The remittance comes backPayment, adjustment, denial and patient responsibility, with the payer's reason and remark codes.
- VixisIQ™Where appropriate data can be obtained and normalized, denials are grouped by payer, programme, site and referral source.
- VixisTrace™One denial is investigated properly, starting with whether it is a documentation problem at all.
- A change worth asking clinicians forOnly once the cause is established, because clinical time is the scarcest resource in the building.
What Should a Programme Ask a Revenue Cycle Partner?
Nine questions for whoever handles your revenue today, and for CareVixis.
-
Do they know the difference between survey readiness and payer criteria?
Ask directly. It is the fastest way to find out whether they have worked with accredited programmes before.
-
Who verifies the benefit captured at intake, and when?
Ask what happens between "insurance captured" and "admission approved", and who is accountable for the gap.
-
Can they report denials by programme and by site?
A single organisation-wide number hides exactly the pattern you need.
-
Do they read payer medical policy?
Ask what a named payer requires for your level of care, and see whether a real answer comes back.
-
What happens the third time the same denial arrives?
Ask for a root cause they found, what changed, and how they knew it worked.
-
How much clinical time will their proposal cost?
Any answer that begins with "document more" should arrive with evidence that documentation is the cause.
-
Will they work inside BestNotes?
Ask what they need from it and what changes if you keep it. CareVixis treats keeping BestNotes as a supported outcome.
-
What will leadership see, and how often?
Then the harder version: what would have told you one referral source brings admissions that consistently do not get paid?
-
Is the work performed in the United States?
Ask about subcontractors as well as staff. CareVixis is 100% US based and does not outsource.
Using BestNotes? Keep It.
Your intake pipeline, admission workflow and survey readiness are built in it. Replacing that is a serious undertaking for any programme, and it would answer none of the questions on this page.
Keeping your system is a supported outcome, not a fallback. If you later want to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis.
Keep BestNotes. Find out what the payer is asking for.
Questions Programmes Ask
Do I have to leave BestNotes to work with CareVixis?
No. Keeping BestNotes is a supported outcome, not a fallback. CareVixis claims no API integration, certification or partnership with BestNotes, and what is agreed before any commitment is which revenue-cycle data can be obtained and how.
What does BestNotes provide?
BestNotes describes an EHR for therapy, behavioral health and addiction treatment, serving solo practices through to enterprise behavioral health organizations, with online intake and prescreening, capture of insurance information prior to admission, admission criteria checklists, and clinical documentation that takes account of Joint Commission, CARF and national, state and county standards.[1]
Why does capturing insurance before admission matter so much?
Because admission is the last cheap moment. Everything after it costs money to deliver. A benefit gap found at intake is a conversation; the same gap found after discharge is a write-off, and the care has already been given.
Does CareVixis decide medical necessity?
No. Admission criteria, level of care and clinical documentation are clinical decisions made by your team against the payer's criteria. CareVixis works on the revenue consequences: what was authorized, what was billed, what came back, and why.
Will CareVixis tell our clinicians how to chart?
No. What CareVixis can do is tell you which payer is denying which service and what that payer's policy asks for, so that any change your clinical leadership chooses to make is aimed at something real rather than at documenting more in general.
