For behavioral health and human services agencies using NetsmartOne Client, Four Funding Sources. Which One Paid?
An agency billing Medicaid, a county contract, a grant and a state programme does not have one revenue question. It has four, and a denied claim is only one of the ways the money goes missing. Keep Netsmart. Find out which of the four is leaking.
Start with the free CareVixis practice assessment. No email required.
Agency Revenue Is Not Practice Revenue.
Grants, contracts, Medicaid, state reporting and cost settlement sit alongside the claims, and each one has its own rules about what counts.
An outpatient billing page does not describe your organisation, so this one does not pretend to.
One Client, Several Funders.
The same service delivered to the same person can be billable to one funding source, another, or none at all, depending on eligibility that day and which programme they were enrolled in.
So Revenue Goes Missing Before It Is a Claim.
A service that was delivered and never billed to anything will never appear as a denial, because it never became a claim.
Nothing rejects. Nothing errors. The money is simply not there.
We Look Before the Claim, Not Only at the Denials.
CareVixis reconciles what was delivered against what was billed and to which funder, so the gap between a service and a claim becomes visible alongside the ordinary denial and remittance review.
Keep Netsmart.
Netsmart is built for organisations like yours and replacing it is not on the table. The platform stays and the billing reconciliation happens beside it.
Keep Netsmart. Find the revenue that never became a claim.
Everything below is the evidence for what you just read.
What Netsmart publishes about its own platform, why multi-funder revenue hides differently from claim revenue, a ten-question framework, and the source for every fact on this page.
Why is agency revenue harder to see than practice revenue?
Because a denial is the most visible failure, and not the most expensive one. A private practice mostly gets paid or gets denied. An agency can also deliver a service that is never billed at all, bill it to the wrong funding source, exhaust a contract ceiling nobody was watching, or report it one way to the state and another way to the payer.
None of those produce a reason code. They produce a smaller number at the end of the quarter, with no single document explaining it.
Netsmart Is Built for the Breadth, Not Just the Clinic
Very few platforms try to cover this range of programmes at once. Everything below is Netsmart's own published description.[1]
| What it does | How Netsmart describes it |
|---|---|
| The platform | CareFabric, described as a connected platform, with myEvolv and myAvatar as electronic health records within it. |
| Programme breadth | Support for addiction treatment, autism programmes, behavioral health, intellectual and developmental disabilities, foster care and early intervention services. |
| CCBHC | Netsmart positions the platform around Certified Community Behavioral Health Clinic interoperability, reporting, compliance and value-based care initiatives. |
| Public sector | Workflows described as scalable for county behavioral health organisations, with cross-programme coordination and operational visibility. |
| Billing | An integrated EHR and billing solution intended to standardise workflows and speed claims processing across a human services portfolio. |
| Revenue services | Netsmart also describes revenue cycle management coaching, including workflows for unbilled services and eligibility automation. |
Notice that unbilled services appear in the vendor's own description of the problem. That is a good sign: it is the failure most agencies underestimate, and the one least likely to be noticed by anyone watching denials.
A Denial Is Loud. Everything Else Is Quiet.
Denials arrive with a date, a code and a queue. They get worked, because they are impossible to ignore. That is exactly why they are rarely where the largest loss sits in an agency.
The quiet failures have no arrival event. A service delivered and never billed does not notify anyone. A unit charged to a grant that should have gone to Medicaid looks like a completed transaction. A contract approaching its ceiling looks fine until the month it does not.
Counting those requires comparing what was delivered against what was billed and against what was funded, per programme, per site, per funding source. It is a reconciliation problem before it is a billing problem.
What has an owner
- The claim that was denied.
- The rejection that bounced today.
- The payment that posted.
- The report due to the state this month.
What frequently has none
- Services delivered but never billed at all.
- Units billed to the wrong funding source.
- A contract or grant nearing its ceiling.
- Whether the state report and the claims agree.
Six Ways Agency Revenue Disappears Without a Denial
Each of these is invisible to a process built around working denials, which is what most revenue cycle operations are built around.
Services delivered and never billed
A session happens, the note is written, and no charge is ever created. There is no rejection, because there was no claim.
Charged to the wrong funding source
A unit that a payer would have covered is absorbed by a grant, or a grant-eligible service is billed and denied. Both look like completed work.
Contract and grant ceilings
Funding that runs out mid-period. The services continue because the need does, and the last weeks are delivered against nothing.
Eligibility churn
Clients move on and off coverage. An agency serving people through instability will always have some of this, and the question is whether anyone catches it before the service.
Programme rules that differ by line
Foster care, early intervention, IDD and clinic services do not share billing rules. A team fluent in one is not automatically fluent in the next.
The state report and the claim disagree
Two systems of record for the same service, maintained by different people, reconciled by nobody until an audit asks.
This is why an agency needs analysis rather than a bigger denial queue. Four of those six never produce a denial at all, so no amount of working denials will surface them.
Where This Fits Around What You Already Run
An illustration of the general flow for an agency keeping Netsmart, not a description of any specific configuration.
- Service delivered and documentedAcross programmes, sites and funding arrangements, in Netsmart.
- Charge creation and funding source assignmentThe step where the quiet failures start: services with no charge, and charges against the wrong source.
- Claims, clearinghouse and payerFor the portion that is claim funded, adjudicated in whole or in part.
- Remittance, contracts and reportsPayments, denials, contract drawdown and state reporting, in different places.
- VixisIQ™Where appropriate data can be obtained and normalized, delivered services are compared against billed services and against funding source, per programme and per site.
- VixisTrace™A failed claim is investigated for a root cause, and an unbilled service is traced to the step that dropped it.
- One reconciled pictureWhat was delivered, what was billed, what was paid, and by whom, with a name against each gap.
How Should an Agency Evaluate a Revenue Cycle Partner?
Ten questions for whoever handles your revenue today, and for CareVixis. A vendor built for private practice will struggle with half of them, which is itself useful to discover early.
-
Can they reconcile delivered services against billed services?
This is the question. If the answer is about denials, they are describing a smaller problem than the one you have.
-
Can they report by programme and by funding source?
An agency-wide total hides exactly the variation you need to act on.
-
Who watches contract and grant ceilings?
Ask who tells you that a funding line is eight weeks from exhausted, and when.
-
Do they understand more than clinic billing?
Ask about the rules for your non-clinical programmes. Foster care and early intervention are not outpatient therapy.
-
How is eligibility churn handled?
Ask what happens between a client losing coverage and the next service being delivered.
-
Do state reporting and billing get reconciled?
Ask who compares them, how often, and what happens when they disagree.
-
What happens the third time the same denial arrives?
Ask for a root cause they found, what changed, and how they knew it worked.
-
Will they work inside Netsmart?
Ask what they need from it and what changes if you keep it. CareVixis treats keeping Netsmart as a supported outcome.
-
What will leadership and the board see?
Then the harder version: what would have told you one programme had been billing nothing for a month?
-
Is the work performed in the United States?
Ask about subcontractors as well as staff. CareVixis is 100% US based and does not outsource.
Using Netsmart? Keep It.
Your programmes, your sites, your compliance obligations and your state reporting are configured in it, across service lines that most software will not touch. Replacing that is a multi-year decision, and it would answer none of the questions on this page.
Keeping your system is a supported outcome, not a fallback. If you later want to move to the CareVixis platform, that move may be offered at no additional migration charge. Keeping the EHR you already have carries no monthly software fee from CareVixis.
Keep Netsmart. Find the revenue that never became a claim.
Questions Agencies Ask
Why is agency revenue harder to see than practice revenue?
Because a denial is the most visible failure and not the most expensive one. An agency can deliver a service that is never billed, bill it to the wrong funding source, exhaust a contract ceiling, or report it one way to the state and another to the payer. None of those produce a reason code.
Do I have to leave Netsmart to work with CareVixis?
No. Keeping Netsmart is a supported outcome, not a fallback. CareVixis claims no API integration, certification or partnership with Netsmart, and what is agreed before any commitment is which revenue-cycle data can be obtained and how.
What is an unbilled service, and why does it matter so much?
A service that was delivered and documented but for which no charge was ever created. It matters because nothing announces it: there is no rejection, no denial and no queue, so it can continue for months while every visible metric looks healthy.
We are a CCBHC. Does that change the analysis?
It changes what the numbers mean rather than whether they are worth having. Under a bundled or prospective rate, the loss usually moves away from denials and towards encounters that were never captured, clients whose eligibility lapsed, and services that belong to a different funding line.
Does CareVixis handle grant reporting or cost reports?
No, and you should be wary of anyone who says otherwise casually. Grant compliance and cost reporting are specialist finance functions. What CareVixis works on is the revenue cycle underneath them: what was delivered, what was billed, what was paid, and why the three do not agree.
