Family Therapy Is Covered When the Patient Is the Point. That Is Where LMFT Claims Are Won or Lost.
Medicare covers family psychotherapy with or without the patient present, as long as treating the patient is the primary purpose. Getting that on the claim and in the note is the difference between a paid session and an appeal. CareVixis handles the billing, the enrollment and the follow-up for marriage and family therapy practices.
Your Session Has More Than One Person In It. Your Claim Has One Patient.
Marriage and family therapy is the one modality where the billing question and the clinical question are the same question: who is being treated, and does the record say so?
Family psychotherapy is covered, with a condition attached
Medicare covers family psychotherapy with or without the patient present, as medically reasonable and necessary, with patient treatment as the primary purpose.2 That last clause is the whole ballgame. A session documented as relationship work rather than treatment of the identified patient is the denial you will spend a month appealing.
You have been able to bill Medicare directly since January 1, 2024
CMS implemented section 4121 of the Consolidated Appropriations Act, 2023: since January 1, 2024, marriage and family therapists may enroll in Medicare and bill it independently.1 For a modality that had been outside Medicare entirely, that is a payer most MFT practices have still not turned on.
The rate is fixed, and you must accept assignment
Medicare Part B pays marriage and family therapists at 75% of what a clinical psychologist is paid under the Physician Fee Schedule, and you must accept assignment.1
Two rules that catch MFT practices
Medicare does not pay under the MFT benefit category for MFT services a hospital outpatient department or community mental health center provides to patients under a partial hospitalization or intensive outpatient program.2 And while your services can be covered as auxiliary personnel incident to a physician or other qualifying practitioner, Medicare does not cover services provided incident to your own professional services.2
Documentation, Coding and Follow-Up That Match How You Actually Work
We are not going to tell you to see fewer families or document defensively. We are going to make sure the claim reflects the treatment you already provided.
Claims, start to finish
Submission, clearinghouse, rejections, denials, appeals and accounts receivable follow-up. Not a portal you log into. Work that gets done.
Credentialing and enrollment
Medicare, Medicaid by state, the commercial payers and the behavioral health carve-outs. Plus CAQH upkeep and expiration tracking, which is where panels quietly lapse.
VixisIQ™ revenue intelligence
What was billed, what was paid, what is outstanding and what is stuck, without assembling it yourself from three systems.
VixisTrace™ root cause analysis
Not just which claims were denied. Why, and what has to change upstream so the same denial stops recurring.
Keep your EHR, or move
If your current system works, keep it and we manage the billing around it. Keeping the EHR you already have carries no monthly software fee from CareVixis.
A US-based team you can reach
100% US-based, zero outsourcing. You talk to the people who work your claims, not a queue in front of them.
