Medicare Pays Your LCSW Claims at 75% of the Psychologist Rate. Everything Else Has to Be Right.
You cannot change the percentage. You can change how much of what you bill actually gets paid, how fast, and how much of your week it costs you. CareVixis runs the billing, the credentialing and the follow-up for clinical social work practices so the claim is correct the first time.
The Rules That Decide Whether an LCSW Claim Gets Paid
Clinical social work has its own Medicare benefit category, with its own payment rate and its own list of settings it does not cover. Most denials we see are not clinical disagreements. They are the claim landing in the wrong category.
Your rate is set by statute, not by negotiation
Medicare pays for clinical social worker services at 75% of the fee schedule amount for clinical psychologists.2 The statute sets it at 80% of the lesser of the actual charge or 75% of that amount.2 That is the ceiling. Everything you recover above what you collect today comes from claims that were denied, underpaid, or never followed up.
Four settings fall outside the CSW benefit category
Medicare does not pay under the CSW benefit category for hospital inpatient services, for services to patients in a partial hospitalization or intensive outpatient program at a hospital outpatient department or community mental health center, or for services to skilled nursing facility inpatients and patients in Medicare-participating ESRD facilities where those services fall under the respective provider’s participation requirements.2 A clinician who works across settings can bill the same CPT code correctly one day and have it land outside the benefit the next.
Hospital outpatient work is covered, and it is billed differently
Medicare covers CSW hospital outpatient services and pays them under the CSW benefit category when the hospital bills under the CSW’s NPI.2 If you split time between a private caseload and hospital outpatient work, the NPI on the claim is doing real work.
Since 2024 you can bill health behavior codes
For CY 2024, CMS allowed clinical social workers, along with marriage and family therapists and mental health counselors, to bill the health behavior assessment and intervention codes (CPT 96156, 96158, 96159, 96164, 96165, 96167 and 96168), which had been limited to clinical psychologists.3 These codes address the psychological and behavioral factors in treating a physical health problem. Many LCSW practices doing exactly that work are still not billing them.
One Team That Knows Which Category Your Claim Belongs In
You are not buying software with a support line. You are handing the whole revenue cycle to a US-based team that does this every day and is accountable for the result.
Claims, start to finish
Submission, clearinghouse, rejections, denials, appeals and accounts receivable follow-up. Not a portal you log into. Work that gets done.
Credentialing and enrollment
Medicare, Medicaid by state, the commercial payers and the behavioral health carve-outs. Plus CAQH upkeep and expiration tracking, which is where panels quietly lapse.
VixisIQ™ revenue intelligence
What was billed, what was paid, what is outstanding and what is stuck, without assembling it yourself from three systems.
VixisTrace™ root cause analysis
Not just which claims were denied. Why, and what has to change upstream so the same denial stops recurring.
Keep your EHR, or move
If your current system works, keep it and we manage the billing around it. Keeping the EHR you already have carries no monthly software fee from CareVixis.
A US-based team you can reach
100% US-based, zero outsourcing. You talk to the people who work your claims, not a queue in front of them.
Frequently Asked Questions
Yes. CareVixis handles the full revenue cycle for clinical social work practices: claim submission, clearinghouse, denial work, appeals and accounts receivable follow-up, along with credentialing and payer enrollment. Solo caseloads and group practices both.
Medicare pays for clinical social worker services at 75% of the fee schedule amount for clinical psychologists, and the statute expresses it as 80% of the lesser of the actual charge or 75% of that amount.2 Commercial payers set their own rates and are not bound by that formula, which is one reason payer mix matters so much to a social work practice.
Hospital inpatient services; services to patients under a partial hospitalization or intensive outpatient program furnished at a hospital outpatient department or community mental health center; and services to skilled nursing facility inpatients and patients in Medicare-participating ESRD facilities where those services fall under the respective provider’s participation requirements.2
Yes, since CY 2024. CMS finalized allowing clinical social workers, marriage and family therapists and mental health counselors to bill CPT 96156, 96158, 96159, 96164, 96165, 96167 and 96168, previously billable by clinical psychologists only.3
A master’s or doctoral degree in social work, at least two years of supervised clinical social work, and licensure or certification in the state where you provide services. In a state without licensure or certification, at least two years or 3,000 hours of post-master’s supervised clinical experience in an appropriate setting.2
Core revenue cycle management is 6% of insurance collections. Nothing on patient payments. VixisConnect™ communications and VixisGrowth™ growth services are +1% each, so all-in is 8%. Keeping the EHR you already have carries no monthly software fee from CareVixis. Month to month, no long-term agreement and no minimum.
