Chiropractic revenue management, for practices on ChiroTouch

ChiroTouch Tells You a Claim Was Denied. Not Why It Keeps Happening.

ChiroTouch closes the billing loop inside itself. Codes move from the SOAP note to the billing screen, claims go out through its clearinghouse, remittance comes back and posts, and reconciliation matches payments to claims and flags what does not line up. That loop is doing real work. What a loop reports is that something is wrong. Naming the cause, working the payer and clearing the aged balance is a different job, and it is the one CareVixis does.

Your Claims Already Go Out Clean.

ChiroTouch builds the charge from the note, scrubs the claim, sends it electronically and posts the remittance when it comes back.

For most claims, on most days, that is the whole story and it works.

Then Reconciliation Flags Something.

A balance is outstanding. A payment does not match the claim. ChiroTouch tells you, which is exactly what it should do.

Now someone has to find out why, call the payer, and decide what to do about the next one.

In Chiropractic, Why Is Rarely a Formatting Problem.

Whether a visit is active treatment or maintenance is a clinical judgement. So is whether the note supports the level of adjustment billed.

No scrubber can check those, because they are not rules about the claim. They are statements about the care.

So the Same Denial Comes Back.

The claim gets reworked. The cause does not get named, so next month it happens again, and the month after that.

Meanwhile the personal injury balances sit quietly, and a twice-reworked claim drifts toward its filing deadline.

Keep ChiroTouch. Hand Over the Rest.

CareVixis manages the revenue cycle around your existing workflow: denials, appeals, A/R, payer follow-up and credentialing.

Nothing clinical changes. Your team keeps the software it already knows.

ChiroTouch runs the practice. CareVixis runs the revenue cycle around it.

The detail

The detail, if you want it

What ChiroTouch publishes about its own billing, where chiropractic claims actually fail, how CareVixis joins the workflow, and what to ask any billing company before you sign with one.

Can a chiropractic practice keep ChiroTouch and outsource its billing?

Yes. Nothing about outsourcing the revenue cycle requires a chiropractic practice to leave ChiroTouch. The practice keeps scheduling, charting and claim creation exactly where its team already knows them. CareVixis works alongside that workflow and manages what happens to the money afterwards: claim follow-up, denials, appeals, accounts receivable, payment posting review, payer escalation and credentialing.

CareVixis is a revenue management company rather than a software vendor, so there is no second clinical system to learn and no migration to schedule. CareVixis has no API integration, native integration, certification or partnership with ChiroTouch and does not claim one. It works from the claim, remittance and revenue cycle data a practice is able to make available, which is the same material any billing partner works from.

Credit where it is due

What ChiroTouch Publishes About Its Own Billing

['Everything in this table is ChiroTouch describing ChiroTouch, quoted from its own product pages and cited below. It is here because a page about the revenue cycle around a system should start by being accurate about the system.', "ChiroTouch's billing layer is called CT MaxClear, and ChiroTouch states plainly that it “is only available to ChiroTouch Advanced customers”. That single sentence matters more to a revenue conversation than most feature lists: two practices can both say they run ChiroTouch and have entirely different claim paths."]

ChiroTouch capability as ChiroTouch publishes it. CareVixis has not independently tested these features and states them as the vendor's own descriptions.
What it doesHow ChiroTouch describes it
Claims and clearinghouseCT MaxClear is “Integrated with the Change Healthcare Clearinghouse” and lets staff “send claims electronically to payers”.
Scrubbing and remittance“Automated claims scrubbing and ERA posting.” Electronic remittance advice files “download automatically in ChiroTouch”.
Documentation to charge“ChiroTouch links the codes in your provider’s SOAP notes to the billing screen,” and patient data “syncs between charting and billing”.
Before submissionCompliance Scan “flags missing documentation before submission”.
ReconciliationIt “simplifies the reconciliation process by automatically matching insurance payments to claims and identifying discrepancies or outstanding balances”.
Posting“Insurance payments automatically and securely post to the patient’s ledger,” and billers “can post payments directly through the ChiroTouch portal”.
Claim tracking“Manage the entire claims cycle, from submitting claims electronically to automatically updating claims status in one interface.”
Which practices have it“CT MaxClear is only available to ChiroTouch Advanced customers.”
ReportingChiroTouch generates “automated clinical, operational, financial, and visit reports”.

CareVixis has not independently tested any of these features and does not restate ChiroTouch's published performance figures anywhere on this page. Where ChiroTouch says it does something, this page says ChiroTouch says it.

The gap

A Closed Loop Reports. It Does Not Investigate.

Reconciliation that automatically matches payments to claims and identifies discrepancies is a genuinely good thing to have. It is also, precisely, a detection layer. It surfaces the outstanding balance and the underpaid line; it does not tell a practice owner why the payer decided that, whether the same decision is being made on other claims, or what to change so it stops.

The claims that survive an automated scrubber are the ones worth understanding. A scrubber encodes rules that can be stated in advance: a missing identifier, a malformed field, a code pairing that is known to be invalid. Denials that get through are the ones no rule could have anticipated, which means their causes live in payer policy, enrollment, documentation judgement and contract terms rather than in the claim's formatting.

That is the gap CareVixis fills. Not a better scrubber. The work that starts after the scrubber has done everything it can.

What ChiroTouch's loop closes

  • Charges built from the SOAP note.
  • Format and code-pairing errors before submission.
  • Electronic submission and claim status in one interface.
  • ERA download and posting to the ledger.
  • Payments matched to claims, with discrepancies flagged.

What a flagged discrepancy does not say

  • Why this payer denied this visit.
  • Whether the same denial is repeating across a payer.
  • Whether the allowed amount matched the contract.
  • Who is working the balance, and when they last touched it.
  • Whether a clinician was enrolled on the date of service.
Where the money actually goes missing

The Chiropractic Denials a Scrubber Cannot Catch

These are specific to chiropractic, and none of them is a formatting problem. Not every practice sees all of them, and a practice may see none. They are the categories CareVixis expects to look at first on a chiropractic account.

Active treatment versus maintenance

The most expensive decision in chiropractic billing is a clinical judgement about the nature of the care, not a field a scrubber can validate. Medicare treats the distinction as the difference between a covered service and a denied one. CareVixis covers the published rule in detail on its chiropractic revenue management page.

The level of the adjustment

98940, 98941 and 98942 describe manipulation of different numbers of spinal regions, and 98943 covers extraspinal regions. The note has to support the level billed. A denial or downcode here is a documentation conversation, not a claim correction.

Medical necessity after improvement

A course of care that was plainly necessary in week one can stop reading that way by week twelve, and the payer is reading the documentation rather than watching the patient. Denials in this category arrive late and in batches.

Visit caps and authorisation limits

Many plans cap visits per condition or per year, or require authorisation past a threshold. A claim can be clean, coded correctly and still fall outside what the plan agreed to cover.

Personal injury and third-party liability

Chiropractic carries an unusually high share of auto, personal injury and workers' compensation cases. Those balances behave nothing like commercial claims: different timelines, different documentation, and long silences that look identical to a lost claim.

Timely filing on a reworked claim

A denial that is reworked twice can quietly age past the payer's filing limit while it looks like it is being handled. The claim was never ignored. It was just never finished.

CareVixis publishes no denial rate, no recovery rate and no resolution time for chiropractic or any other specialty, and does not claim any of these categories is common. What a particular practice is actually experiencing is a question for its own data.

Conceptual workflow

Where This Fits Around What You Already Run

Where a chiropractic claim goes on ChiroTouch, and where CareVixis joins it. The first five steps are the practice's existing workflow and do not change.

  1. Visit and documentationThe encounter is charted in ChiroTouch, and the codes in the SOAP note link through to the billing screen.
  2. ChiroTouch claim creationCharges are built, and Compliance Scan flags missing documentation before submission.
  3. ClearinghouseOn the Advanced tier, CT MaxClear submits electronically through the Change Healthcare clearinghouse. CareVixis has documented that clearinghouse separately.
  4. Payer adjudicationThe claim is paid, adjusted, denied or rejected before adjudication. A rejection never became a claim, so there is nothing to appeal.
  5. Remittance returnsERA files download automatically and post to the ledger, and reconciliation flags discrepancies and outstanding balances.
  6. VixisIQ™CareVixis revenue intelligence reads the resulting remittance information, where appropriate data can be obtained and normalized, and looks at A/R, payer behaviour and denial patterns over time rather than claim by claim.
  7. VixisTrace™When a claim has failed, CareVixis claim root cause analysis runs it through eight stages: Detect, Investigate, Identify, Correct, Prevent, Resubmit, Verify, Monitor.
  8. The CareVixis teamReal people work the appeal, the payer call, the aged balance and the enrollment problem. Denials are worked the day they arrive; submitting the corrected claim or appeal the same day is the CareVixis standard.
A framework you can use on anyone

What to Ask Any Billing Company That Says It Works With ChiroTouch

Ask CareVixis these too. A practice that asks all nine of a shortlist will learn more from the differences in the answers than from any page like this one.

  1. Are you on the Advanced tier, and do they know it?

Ask the question of yourself first. Whether CT MaxClear is in play changes which clearinghouse your claims move through and what your billing partner is actually looking at.

  2. What do you do with the denials the scrubber did not prevent?

Working them is the minimum. Ask what happens the second time the same denial appears from the same payer.

  3. Who decides whether a visit is active treatment?

The answer should be that the clinician decides and the documentation supports it. A billing company that offers to make that call for you is offering the wrong thing.

  4. How would a downcode from 98941 to 98940 ever be noticed?

It is paid, so it does not appear on a denial report. Ask for the mechanism that catches a paid claim that was paid at the wrong level.

  5. Are payments checked against contracted rates?

An underpayment is not a denial and will not show up as one. Ask how a quiet shortfall on a routine code would surface.

  6. How do you handle personal injury and auto balances?

Ask specifically, because these age differently and a general A/R process tends to leave them alone until they are dead.

  7. What happens to a claim that has already been reworked twice?

Ask who owns the filing deadline and how you would know a claim was approaching it.

  8. Do you handle credentialing and enrollment?

Enrollment effective dates cause denials that look like coding problems. Ask whether the same partner sees both.

  9. What will you show me, and how often?

Ask what reporting looks like and what it will say when the news is bad. A partner who only reports good months is not reporting.

Using ChiroTouch? Keep It.

Your team knows this software. Scheduling, documentation and billing are already connected in it, the codes already flow from the note to the claim, and that integration is doing real work every day. A migration would answer none of the questions on this page.

Keeping your system is a supported outcome, not a fallback. CareVixis manages the revenue cycle around whatever a practice already runs. If a practice later wants to move to the CareVixis platform, that move may be offered at no additional migration charge, and everything the patient sees still carries the practice's own name and branding.

Keep ChiroTouch. Explain the claims it could not scrub.

Questions Chiropractors Ask About ChiroTouch and Billing

Does CareVixis work with chiropractic practices using ChiroTouch?

Yes. CareVixis provides chiropractic revenue cycle management for practices that use ChiroTouch. The practice keeps ChiroTouch and CareVixis manages billing, denials, accounts receivable, payer follow-up and credentialing around it. CareVixis is not affiliated with, endorsed by, sponsored by or certified by ChiroTouch.

Do I have to replace ChiroTouch to work with CareVixis?

No. ChiroTouch does not need to be replaced for a practice to use CareVixis revenue cycle services. Scheduling, charting and claim creation stay where your team already knows them.

Does CareVixis replace my EHR?

Not unless you want it to. CareVixis publishes its own practice platform, VixisCare™, and a practice may move to it later. On this page the assumption is that you are keeping ChiroTouch, and that is a fully supported arrangement rather than a lesser one.

Can CareVixis work with my existing clearinghouse?

Yes. Where appropriate data can be obtained and normalized, CareVixis works from the claim and remittance information a practice is able to make available, whichever clearinghouse produced it. ChiroTouch states that CT MaxClear is integrated with the Change Healthcare clearinghouse, which CareVixis has documented separately.

What is VixisIQ™?

VixisIQ™ is the CareVixis revenue intelligence technology. It analyses revenue cycle and remittance data to surface payment patterns, adjustments, denial and rejection patterns, payer behaviour and accounts receivable trends. What it can analyse for a particular practice depends on the data that practice is able to make available.

What is VixisTrace™?

VixisTrace™ is the CareVixis claim root cause analysis technology. It covers rejected, denied, underpaid and otherwise problematic claims across eight stages: Detect, Investigate, Identify, Correct, Prevent, Resubmit, Verify and Monitor. The objective is continuous improvement: identify preventable patterns, address their causes, and reduce unnecessary repeat failures.

How does CareVixis handle denied chiropractic claims?

Denials are worked the day they arrive, and submitting the corrected claim or appeal the same day is the CareVixis standard. Beyond the individual claim, VixisTrace™ looks for the cause behind a repeating denial so the practice is not reworking the same failure every month.

Can CareVixis work old chiropractic accounts receivable?

Yes. Legacy accounts receivable is a separate arrangement and is charged on what CareVixis actually recovers rather than on the balance handed over. The terms are published on the pricing page.

Can CareVixis help with chiropractic credentialing?

Yes. VixisCred™ handles payer enrollment and credentialing, which matters here because enrollment effective dates produce denials that look like coding problems. See the pricing page.

How do I know whether my practice has a revenue cycle problem?

Look at your own numbers before talking to anyone. The free practice assessment asks five questions and returns what it finds, with no email address required and no obligation.
