For practices submitting through the Claim.MD portalYour Rejections Come Back Fast. Does Anyone Count Them?
Claim.MD is unusually good at handing a rejected claim straight back to you, quickly, with enough detail to correct it. That makes each rejection cheap. It is also the reason nobody ever adds them up.
Start with the free CareVixis practice assessment. No email required.
Fast Rejections Are a Real Advantage.
Claim.MD hands a rejection back quickly, your biller corrects it and it goes out again the same day.
That is a good system being used well, and plenty of practices do not have it.
Fixing Is Not the Same as Counting.
Each one gets corrected and resubmitted. Almost nowhere does anybody write down why it bounced in the first place.
So It Comes Back Next Month.
Forty corrections in a quarter feels like forty small pieces of work. Counted by cause, it is often three problems that nobody has been asked to fix.
The speed that makes the correction easy is also what makes the pattern invisible.
We Count the Denials and the Rejections.
CareVixis records a cause for every Claim.MD rejection and every payer denial, then reports which causes repeat and how often, so the billing fix moves upstream of the correction.
Keep Claim.MD.
The transport stays, the speed stays, and the billing workflow your biller likes stays. What gets added is a record of what all that speed has been handing back.
Keep Claim.MD. Start counting what it hands back.
Everything below is the evidence for what you just read.
What Claim.MD publishes about its own service, why a corrected rejection still costs you, a ten-question framework you can put to any billing company, and the source for every fact on this page.
Correcting a Rejection and Understanding It Are Two Different Jobs
Claim.MD says on its own site that returning rejected claims for correction is the feature its providers value most, because it speeds payment and removes the confusion of re-billing. That is a real strength and it is worth keeping.
Follow the consequence, though. A rejection that takes ninety seconds to fix never becomes a ticket, never gets escalated, never reaches a meeting and never gets counted. It is absorbed. The same payer can reject the same way forty times in a quarter, and the practice experiences it as forty small interruptions rather than one fixable problem.
CareVixis does not replace your clearinghouse and does not slow that loop down. VixisTrace™ records why a claim failed and categorizes it so that recurrence becomes visible, and where appropriate data can be obtained and normalized, VixisIQ™ reads the remittance information across months. Your staff keep correcting claims exactly as fast as they do now.
What Claim.MD Publishes About Its Own Clearinghouse
Everything below is Claim.MD's own published description, summarised from claim.md and cited at the foot of this page. It is listed because the correction loop described in it is genuinely good, and because this page argues for adding to it rather than replacing it.
| What it does | How Claim.MD describes it |
|---|---|
| The portal | A medical clearinghouse portal where claims can be created directly, or uploaded from any medical billing system. |
| Claim entry and formats | CMS-1500 and UB-04 style claim entry with validation, single hand-entered claims or large batch files, and uploads in 837P, 837I, 5010, 4010, CSV, XLS, XML, NSF and print image formats. |
| Rejected claims | Claim.MD describes returning rejected claims for corrections as the feature its providers most value, speeding payment and eliminating the confusion of re-billing. |
| Remittance | Download, print or view electronic payment detail, including standard 835, spreadsheet and XML remittance data, or a PDF of payment details. |
| Eligibility | "Real time eligibility" for payers, described as reachable through the web portal and through an API. |
| Support | A GetHelp ticket can be opened from a claim, an ERA or an eligibility record, handled by a United States based support team. |
| Plans | A monthly subscription with unlimited claims and ERA, a small volume plan described as suited to single doctor practices, and a basic plan where you pay only for what you use. |
Notice the fourth row. Claim.MD will hand you remittance data as a spreadsheet or as XML, which is more than many networks offer. The data is not the constraint in this practice. Whether anybody opens twelve months of it together is the constraint.
A Ninety Second Fix Is Cheap. That Is Exactly Why It Never Gets Counted.
A rejection corrected in the portal produces two things: a paid claim, and no record of why it was rejected. The work happened, the money arrived, and the cause was never written down anywhere that anybody will read it again.
Speed of correction and visibility of pattern are not the same property, and here is the uncomfortable part: fast correction can actively hide the pattern. A slow, painful rejection process at least generates complaints, and complaints eventually get investigated. An efficient one generates silence.
So the practice keeps paying a small tax forever. Every one of those corrections was avoidable further upstream, in an enrollment that was never finished, an eligibility response nobody compared to the claim, or a field being populated the way one payer wants and another does not.
What a fast correction loop gives you
- The claim is fixed today instead of next month.
- Payment arrives sooner on that claim.
- No confusion about whether to re-bill.
- Your staff are not fighting the tool to get work done.
What it quietly costs you
- Nobody knows how many rejections there were.
- Nobody knows which payer produced most of them.
- Nobody knows which of them were the same cause repeating.
- Nothing upstream ever changes, so next quarter looks the same.
Six Things a Correction Queue Will Never Tell You
Each of these is answered by counting and investigating, not by correcting faster. None of them is a criticism of the portal.
A fix that leaves no trace
The claim is corrected and paid. The reason it failed exists only in the memory of whoever fixed it, and only until the next one arrives.
The same rejection, forty times
Forty identical rejections spread across a quarter feel like forty small interruptions. Counted together they are one problem with one upstream cause, and that cause can be addressed.
An export you can read and nobody does
Standard 835, a spreadsheet and XML are all available. A file that can be opened is not the same as a file somebody opens on a schedule and compares to last month.
Rejections and denials treated as one chore
A rejection failed before adjudication and was never processed as a claim, so there is nothing to appeal. A denial is a payer decision after adjudication. Different causes, different remedies, one queue.
Underpayments that arrive as payments
Nothing rejects and nothing denies. The money simply comes in lower than the contract implies. A workflow built on exceptions never sees it, because there was no exception.
Eligibility checked, benefits never reconciled
An eligibility response before the visit and a remittance after it are two separate records. Whether what was quoted matches what was paid is a question somebody has to ask deliberately.
All six are answered by counting and investigating rather than by correcting. The objective is continuous improvement: identify preventable patterns, address their causes, and reduce unnecessary repeat failures.
Where This Fits Around What You Already Run
An illustration of the general flow for a practice keeping Claim.MD, not a description of any integration. CareVixis claims no connection to Claim.MD.
- The claim is created or uploadedIn the portal directly, or from whatever billing system your practice already runs.
- Validation and transmissionThe claim is checked and sent on to the payer through the network.
- Rejected before adjudicationHanded back quickly for correction. This is the part that already works well.
- Adjudicated, then paid, adjusted or deniedThe payer decides, and the decision returns with its own reason codes attached.
- Remittance retrievedAs a standard 835, a spreadsheet, XML or a PDF of the payment detail.
- VixisTrace™The reason a claim failed is recorded and categorized, so that the fortieth occurrence is visible as the fortieth rather than as a fresh surprise.
- VixisIQ™Where appropriate data can be obtained and normalized, the remittance information is analyzed across payers and months.
What to Ask a Billing Company When You Already Run Claim.MD
Nine questions for whoever handles your revenue today, and for CareVixis. The first two separate a correction service from a revenue management one.
-
Do they count rejections, or only clear them?
Ask for the number for last quarter. If nobody can produce it, nobody is counting it.
-
Do they separate rejections from denials?
Different causes and different remedies. Anyone who says denial for both has not looked closely at either.
-
What happens to the reason, not just the claim?
Correcting the claim is the minimum any service does. Ask what is recorded about why it failed, and who reads it.
-
Will they use the remittance exports you already have?
Standard 835, spreadsheet and XML are available to you. Ask which they would read, and how often.
-
How far back will they look on day one?
Ask for a specific period. An answer of as far back as possible is not a period.
-
Who keeps the portal login, and who is accountable?
Either answer can work. The failure mode is when neither side has written it down and both assume the other.
-
Can they tell you which payer causes most of your rework?
This is the question a correction queue cannot answer and an analysis can. Ask how they would produce it.
-
Do they look for underpayments, not just denials?
A payment lower than the contract implies is invisible to any process built on exceptions. Ask what makes it visible.
-
What will you see, and when?
A named output on a named cadence. An answer with no date in it is not an answer.
Using Claim.MD? Keep It.
Your claims go out through Claim.MD, your rejections come back through Claim.MD, your staff know the portal and your plan suits your volume. That is a working arrangement, and CareVixis does not ask you to change any of it.
Keeping your system is a supported outcome, not a fallback. What is added is a count and a cause: how often each failure happens, what is producing it, and whether it stops once somebody addresses it.
Keep Claim.MD. Start counting what it hands back.
Questions Practices Ask About Claim.MD and Rejected Claims
Does CareVixis replace Claim.MD?
No. Claim.MD is a clearinghouse that transmits your claims and returns your remittance information, and CareVixis does not ask a practice to leave it. Analyzing what comes back is a different job from moving the claim.
If our rejections are already easy to fix, what is the problem?
Nothing is wrong with the fix. The problem is that an easy fix is never counted, so a cause that produces forty rejections a quarter is experienced as forty small interruptions instead of one thing that could be addressed once.
What is the difference between a rejection and a denial?
A rejection fails before adjudication, at the clearinghouse or the payer front end, and was never processed as a claim, so there is nothing to appeal. A denial is a payer decision made after adjudication. The causes differ and so do the remedies.
Can you work from the remittance exports we already have?
That is usually the practical route. Claim.MD publishes standard 835, spreadsheet and XML remittance downloads. What CareVixis can analyze for a particular practice depends on the data that practice is able to make available, which is confirmed before any commitment.
Do we have to change our plan or our workflow?
No. Your submission route, your portal and your correction loop stay as they are. What changes is that the reason a claim failed is recorded and categorized rather than absorbed and forgotten.
What does claim root cause analysis actually produce?
A recorded cause for a failed claim, a category it belongs to, a corrective action, and tracking that shows whether the same failure returns. The objective is continuous improvement: identify preventable patterns, address their causes, and reduce unnecessary repeat failures.
Keep Claim.MD. Add Revenue Intelligence.
Keep the clearinghouse and keep the fast correction loop. Add the one thing it does not give you, which is the number, the cause and whether the cause was ever addressed.
