Chiropractic revenue management, for practices on zHealth

You Are Good at Getting Them Back. Are the Visits Still Getting Paid?

zHealth is built around retention: recall, reminders, reviews, texting, memberships. It is good at the thing chiropractic depends on, which is the patient who returns. The awkward truth underneath is that the more often a patient returns for the same complaint, the closer each visit moves to the line where a payer stops calling it treatment.

You Are Good at Getting Them Back.

Recall, reminders, reviews, texting, memberships. zHealth is built around the returning patient.

In chiropractic that is the whole economy.

Payers Look at the Same Visits Differently.

Coverage turns on whether care is still active treatment or has become maintenance.

A long course of care for one complaint is where that question gets asked.

Templates Are Efficient. They Are Also Repetitive.

A note that produces itself quickly can end up describing the same thing every week.

If the record stops showing change, the course of care gets hard to defend.

And Small Errors Compound at Volume.

A routine code billed slightly wrong is trivial once.

Across a year of repeat visits it stops being trivial.

Keep zHealth. Hand Over the Rest.

CareVixis manages denials, appeals, accounts receivable, payer follow-up and credentialing around your existing workflow.

How you practise stays entirely yours.

zHealth brings them back. CareVixis keeps the visit payable.

The detail

The detail, if you want it

What zHealth publishes about itself, why retention and coverage pull against each other, where repeat-visit chiropractic loses money, and what to ask any billing company.

Why does a high return rate create billing risk in chiropractic?

Because chiropractic coverage turns on whether care is still active treatment rather than maintenance, and a long course of care for one condition is exactly where that question gets asked. It is not a reason to see patients less. It is a reason for the documentation and the claim to keep pace with the care.

CareVixis manages that back end while zHealth keeps doing what it is good at. CareVixis has no API integration, native integration, certification or partnership with zHealth and claims none.

['zHealth describing zHealth, quoted from its own site and cited below.', 'Its own summary of what it is for is the useful part: software “specifically designed for chiropractors” that helps “increase new patients”, “improve patient retention”, “automate clinic workflow” and “enhance patient experience”.']

The gap

Retention and Coverage Pull in Opposite Directions

A recall engine is designed to maximise the number of times a patient comes back. Payer policy for chiropractic is designed around the idea that care should be resolving something, and that once maximum therapeutic benefit is reached, continued care is maintenance.

Nothing about that makes retention wrong. Plenty of returning care is genuinely active treatment, and plenty of it is legitimately cash or membership work that no payer is involved in. The failure mode is the middle: visits billed to insurance, month after month, with documentation that has stopped describing progress because the template stopped asking.

The other half is volume. A retention-led practice runs a lot of low-value repeat claims, and a small systematic error on a routine code costs far more at that frequency than it would anywhere else.

What recall is measured on

  • Visit frequency.
  • Rebooking rate.
  • Retention over months.
  • Reactivated patients.

What the payer is looking at

  • Whether the note still shows progress.
  • How long this course of care has run.
  • Whether the plan caps visits.
  • Whether the claim declares active treatment.
Where the money actually goes missing

Where Repeat-Visit Chiropractic Loses Money

These scale with visit frequency, which is exactly what a retention platform is designed to raise.

Maintenance, declared or assumed

Chiropractic coverage turns on a clinical determination the claim has to carry. CareVixis sets out the published rule on its chiropractic revenue management page.

Documentation that plateaus

A template that produces an efficient note can produce the same note. If the record stops showing change, a long course of care becomes hard to defend.

Caps reached mid-course

A plan capping visits per condition is hit sooner by a practice that rebooks well, and the front desk usually learns about it from a denial.

Small errors at high frequency

A routine code billed slightly wrong is a rounding error once and a serious number across a year of repeat visits.

Membership and insurance overlap

Where a plan and a benefit could both cover a visit, the boundary needs to be deliberate rather than whichever was easier to record.

Reactivated patients, stale coverage

A patient recalled after a year may have changed employer, plan or both. The recall succeeded; the eligibility did not.

CareVixis publishes no denial rate, recovery rate or resolution time, and does not suggest any practice experiences all of this. What is happening in a particular practice is a question for its own data.

Conceptual workflow

Where This Fits Around What You Already Run

Where a chiropractic claim goes in a zHealth practice, and where CareVixis joins it. The first four steps are the practice’s existing workflow and do not change.

  1. Recall brings the patient backReminders, recall and texting fill the appointment.
  2. Visit and SOAP noteThe encounter is documented and the charge produced in the same platform.
  3. Claim submittedThe claim leaves for the clearinghouse and the payer.
  4. Payer decisionPaid, adjusted, denied, or rejected before adjudication. A rejection never became a claim, so there is nothing to appeal.
  5. VixisIQ™CareVixis revenue intelligence reads the resulting remittance information, where appropriate data can be obtained and normalized, and watches denial patterns across a course of care rather than one visit at a time.
  6. VixisTrace™Where a visit did not pay, CareVixis claim root cause analysis works eight stages: Detect, Investigate, Identify, Correct, Prevent, Resubmit, Verify, Monitor.
  7. The CareVixis teamPeople work each appeal, each payer call and each balance that stopped moving. Denials are worked the day they arrive; submitting the corrected claim or appeal the same day is the CareVixis standard.
A framework you can use on anyone

What to Ask a Billing Company If You Run a Recall-Led Practice

Put these to CareVixis as well. A high-frequency practice has different exposures from a low-frequency one.

  1. How do you watch a course of care, not just a claim?

Ask what they look at across a patient’s whole episode. Visit-by-visit review misses the pattern that actually gets denied.

  2. Would you tell me my documentation had plateaued?

It is an uncomfortable thing for a vendor to say. Ask whether they would say it.

  3. Who checks eligibility on a reactivated patient?

A recalled patient may have changed plans since their last visit. Ask where that check happens.

  4. How do you spot a small error repeating?

At high frequency the cost is in repetition rather than in any one claim. Ask what surfaces it.

  5. Do you track visit caps per plan?

Ask whether anyone warns the front desk before a limit is reached, or only after the denial.

  6. Where does membership work sit?

Ask how they keep plan services and billed services cleanly separated.

  7. What happens to a denial the second time?

Reworking it is the minimum. Ask what changes so there is no third.

  8. Do you handle credentialing?

Enrollment effective dates produce denials that read as coding problems, so there is value in one partner seeing both.

  9. Can we talk to the people doing the work?

Ask whether you get named contacts or a ticket queue. CareVixis is 100% US based and does not outsource.

Using zHealth? Keep It.

Your recall, reminders, reviews, texting, notes and memberships all run in it and your team knows how. Nothing here argues for changing that.

Keeping your system is a supported outcome, not a fallback. CareVixis manages the revenue cycle around whatever a practice runs. If a practice later wants to move to the CareVixis platform, that move may be offered at no additional migration charge, and everything the patient sees still carries the practice’s own name and branding.

Keep zHealth. Keep the returning visit payable.

Straight answers

Questions Chiropractors Ask About zHealth and Billing

Does CareVixis work with practices using zHealth?

Yes. CareVixis provides chiropractic revenue cycle management for practices on zHealth. The practice keeps zHealth and CareVixis manages denials, accounts receivable, payer follow-up and credentialing around it.

Do I have to replace zHealth to work with CareVixis?

No. zHealth does not need to be replaced. Scheduling, recall, documentation, memberships and payments stay exactly where they are.

Does seeing patients frequently cause denials?

Not in itself. Chiropractic coverage turns on whether care is active treatment rather than maintenance, and a long course of care for one condition is where payers ask that question. The documentation and the claim need to keep pace with the care.

Will CareVixis tell me to see patients less?

No. Clinical decisions belong to the clinician. CareVixis manages the revenue cycle and will tell you what the payers are doing with the claims, which is a different question from how you practise.

What is VixisIQ™?

VixisIQ™ is the CareVixis revenue intelligence technology. It analyses revenue cycle and remittance data to surface payment patterns, adjustments, denial and rejection patterns, payer behaviour and accounts receivable trends. What it can analyse for a particular practice depends on the data that practice is able to make available.

What is VixisTrace™?

VixisTrace™ is the CareVixis claim root cause analysis technology, covering rejected, denied, underpaid and otherwise problematic claims across eight stages. The objective is continuous improvement: identify preventable patterns, address their causes, and reduce unnecessary repeat failures.

Can CareVixis work balances that have been sitting a while?

Yes, as a separate arrangement charged on what is actually recovered rather than on the balance transferred. The terms are on the pricing page.

Is credentialing part of what CareVixis does?

Yes, through VixisCred™. Enrollment effective dates are a frequent hidden cause of denials that present as coding errors. See the pricing page.
